Wellsential Health is currently seeking a Clinical Liaison to support referral development, hospital relationships, and admissions coordination for our McAllen/Edinburg-area skilled nursing facilities.

What You'll Do

As a Clinical Liaison, you will play a key role in supporting census development and helping patients transition from hospitals and other referral sources into the appropriate skilled nursing facility. This role serves as a bridge between external referral partners, patients, families, facility teams, and central admissions.

The Clinical Liaison will spend most of their time in the field building relationships, completing referral screenings, and coordinating admissions with facility and regional teams.

Key responsibilities include:

  • Build and maintain strong relationships with hospitals, case managers, social workers, physicians, discharge planners, home health agencies, assisted living centers, and other referral partners
  • Complete daily sales calls, hospital rounds, and account visits to identify referral opportunities and support census growth
  • Conduct on-site referral screenings to assess patient care needs and determine appropriate post-acute placement
  • Review clinical, financial, insurance, service availability, and bed availability information to help identify the most appropriate facility placement
  • Respond timely to referral inquiries from hospitals and assigned accounts regarding potential admissions
  • Coordinate admissions by communicating referral details with case managers, Central Admissions, facility admissions teams, Administrators, Directors of Nursing, and other appropriate team members
  • Obtain accurate clinical documentation, discharge planning information, and reimbursement-related details needed to support admission decisions
  • Assist patients and families with understanding post-acute care options within Wellsential Health facilities or ancillary service lines when applicable
  • Complete required referral screening forms, daily reports, customer service follow-up documentation, and other assigned reporting
  • Support service recovery by communicating concerns or complaints to appropriate facility, regional, and corporate teams
  • Participate in territory planning, marketing visits, and market development efforts with facility, regional, and business development leadership
  • Share feedback with leadership regarding referral trends, provider feedback, competitor activity, and market changes
  • Maintain HIPAA confidentiality standards and stay informed on skilled nursing regulations, facility ratings, healthcare trends, payer changes, and referral source needs
  • Participate in after-hours or weekend on-call responsibilities as assigned
  • Assist with other duties or projects as assigned
What We're Looking For
  • Active RN, LVN, RRT, or RT license/credential preferred
  • Strong clinical assessment skills required
  • Prior hospital, ICU, LTACH, skilled nursing, rehab, or post-acute care experience preferred
  • Experience working with hospital case managers, discharge planners, physicians, or referral sources is a plus
  • Strong communication, relationship-building, and follow-up skills
  • Ability to assess patient needs and help identify appropriate post-acute placement
  • Strong organization skills and ability to manage multiple referrals, accounts, and priorities
  • Comfortable working independently in the field and traveling between assigned accounts
  • Ability to maintain professionalism when communicating with patients, families, referral sources, and internal teams
  • Must be familiar with or willing to learn skilled nursing regulations, facility services, payer requirements, and admission guidelines
  • Must be eligible to work in the United States

Qualifications
