This social worker is a member of a passionate multidisciplinary team providing comprehensive care for pediatric and adult patients in the Cerebral Palsy program. Responsibilities include assessment & intervention for barriers to care, acute safety concerns, transition-to-adulthood planning, patient and family coping/stress, and general support as families navigate the healthcare and community systems. As the SW Clinic Leader, they are an active member of the CP Leadership Team, supporting QI initiatives and program development. Additionally, the Clinical Leader provides direct supervision to other social workers within the department and contributes to SW department leadership initiatives.

- Models and leads the Clinical Medical Social Worker within the interdisciplinary team to improve or maintain social, emotional, functional, and physical health of patients and families.
- Ensures ethical and sound decision-making, appropriate documentation, and appropriate interpersonal communication of the social workers for whom they are responsible.
- Collaborates with Clinical Medical Social Work leadership, internal stakeholders and other affiliated teams in the development, implementation, and evaluation of Clinical Medical Social Work services.
