Ampla Health is seeking a compassionate and patient-focused Clinical Case Manager (LVN) to support care coordination, case management, and patient advocacy services. This role works closely with medical providers, behavioral health clinicians, pharmacies, insurance companies, and community organizations to ensure patients receive comprehensive, coordinated care.
Responsibilities
- Provide case management and care coordination services.
- Process and track medication prior authorizations and appeals.
- Coordinate referrals and connect patients with community resources.
- Collaborate with medical and behavioral health teams to support patient care plans.
- Maintain accurate documentation and ensure patient confidentiality.
- Advocate for patient needs and assist with navigating healthcare services.
- Participate in care team meetings and quality improvement activities.
Qualifications
- Current California LVN or RN license required.
- Current CPR certification required.
- Experience working with LGBTQ+ communities required.
- Case management experience preferred.
- Experience with prior authorizations preferred.
- Strong communication, organizational, and patient advocacy skills.
- Bilingual English/Spanish, English/Punjabi, or English/Hmong preferred.
