Full Circle Health is looking for a Clinical Case Manager to support our Raymond Clinic.
Purpose
The purpose of the Clinical Case Manager is to work collaboratively with the health care provider(s), interdisciplinary team, patients and community health partners to coordinate care for individuals with chronic or complex conditions, or those requiring high levels of care coordination. The Clinical Case Manager is responsible for identifying, screening, assessing, planning, implementing, coordinating, monitoring, and evaluating the healthcare services provided to patients at Full Circle Health.
Schedule
8:00am – 5:00pm, Monday - Friday daytime shifts.

Education:
• A.D.N. or B.S.N. from accredited College of Nursing, or a Masters in Social work from an accredited university.
Experience:
• Minimum of two years of clinical practice; previous population health, case management or transitions of care experience preferred.
• Pediatrics experience preferred but not required
Licensure / Certification:
• Valid license to practice as a Registered Nurse (RN) or LPN or Master's Prepared Social Worker in the state of Idaho, current BCLS certification, and Certified Case Manager certification preferred.

Skills Required
