The Opportunity: Under the general supervision of the CCBHC Manager, the Client Access Specialist (CAS) provides time-limited and ongoing recovery focused case management services for newly opened clients of the organization. The CAS is a service broker, coordinator, monitor and planner of services, offering practical help, building community support, teaching recovery-based skills and providing active outreach, advocacy and crisis assistance based on the strengths, needs, abilities, and preferences of the individuals.


What You’ll Do: 

  • Engage individuals newly referred to Community Support Services for the initial 90 days of service.
  • Monitor and coordinate delivery of necessary evaluations (e.g., psychiatric, medical, vocational), surveys, and assessments in order to identify each person’s needs.
  • Coordinate and participate in the development and updates of the treatment plan to include services and assistance necessary to achieve specific objectives, as well as an explanation of the availability, intensity and duration required of each service.
  • Assist clients with applying for and establishing needed benefits and housing programs.
  • Coordinate with regional team staff to transition clients from the care of the CAS to a regional team CRS as appropriate.
  • Coordinate with Intake & Access Manager to refer individuals who are unable to be located or engaged to appropriate engagement services.
  • Assist clients on Assisted Outpatient Treatment (AOT) to attend New Day Court hearings and complete recommended program requirements.
  • Assist individuals to identify and minimize the effects of psychiatric symptoms, enabling individuals to self-manage their illness.
  • Assist individuals to develop/increase interpersonal and community coping skills individually or in groups.
  • Review and monitor services and activities and note progress.
  • Available to the individual to provide, coordinate and/or assist in crisis intervention and stabilization services during regularly scheduled working hours.
  • Assist the individual in achieving the objectives in the ISP and maximizing independence and productivity through support and training in the use of personal and community resources.
  • Assist the individual in increasing social support networks of relatives, friends, and/or volunteers that ameliorate life stresses resulting from the individual’s disability.
  • Accompany individuals to appointments and activities and assist in daily living skills training as needed.
  • Provide rehabilitation activities in community settings appropriate to the needs of the individuals.
  • Maintains required agency and departmental productivity standards.
  • Incorporate the principles of being trauma informed in all aspects of the position.
  • Maintain relevant documentation and provide statistical reports in accordance with agency documentation standards.
  • Attend regularly scheduled staff and supervisory meetings at Community Support Services.
  • Responsible for being knowledgeable about the rights of individuals and procedures governing them.
  • Observe all rules of confidentiality as it relates to clinical information both internally and in dealing with outside individual(s) and/or agencies.
  • Participate in ongoing agency and staff training development programs and become familiar with all relevant agency policies and procedures.
  • Assume other duties and responsibilities as assigned by supervisor, which may include serving on various agency committees.

What We’re Looking For: 

  • Minimum of Associates Degree in a Human Service field required.
  • Prior experience in case management working with individuals with severe and persistent mental illnesses preferred.
  • A car and a valid Ohio driver’s license and insurability through Agency’s insurance carrier. Ability to work a flexible work schedule.
  • LSW Licensure Preferred
