Job Description:

Bring comfort where it matters most. Elysian Hospice is hiring a compassionate Chaplain to provide spiritual care to terminally ill patients and their families in the home setting. Qualifications

Essential Functions

  • Assess the hospice patient and support system to identify spiritual needs, as evidenced by the Plan of Care (POC), documentation in the clinical records, and IDG/IDT conference minutes; provide support for the spiritual needs of the patient/family
  • Attend IDG/IDT meetings and participate in the patient care planning process; participate in the Quality Assessment Performance Improvement (QAPI) process as required
  • Use effective interpersonal relations and communication skills, and facilitate the use of these skills by other team members to achieve desirable outcomes
  • Make reasonable efforts to facilitate visits from local clergy, pastoral counselors, and other individuals who can support the patient's spiritual needs
  • Meet mandatory continuing education requirements of the hospice
  • Demonstrate commitment, professional growth, and competency by maintaining a working knowledge of public and private eligibility standards and requirements
  • Promote the hospice's philosophy and administrative policies to ensure quality of care
  • Demonstrate understanding of coordination and continuity of care by collaborating with the IDG/IDT
  • Accept clinical assignments that are consistent with education and competence to meet the needs of the patients/families

Qualifications

  • Working knowledge of community spiritual and other support resources
  • Strong interpersonal and communication skills
  • Reliable transportation, valid driver's license, and current auto liability insurance (if using a personal vehicle for work)
  • Ability to travel locally to patient homes and healthcare facilities
  • Master of Divinity (MDiv) preferred
  • One year of experience as a Chaplain in a healthcare institution preferred
  • CPR certification preferred (per hospice requirement)
