Schedule outpatient primary care and specialty appointments from ED/Urgent Care, document in medical record and scheduling system, follow up on referrals, track high-priority/intensive cases, and coordinate with nursing to ensure continuity of care.

Responsible for scheduling resources to outpatient primary care or specialty offices.  Accurately documents in the medical record and scheduling system.  Schedules appointments across the Medical Group and follows up on referrals to ensure continuum of care.  Works collaboratively with nursing coordinator to coordinate care.  Serves all of SJH Medical Group.

  • Works on site in the ED, Urgent Care or other designated location to schedule patients with outpatient primary care or specialty office.
  • The measure of success is # of appointments scheduled directly from the ED, Urgent Care or other designated location prior to discharge.
  • Tracks high priority appointments and follows up on all patients designated as needing intensive tracking.
  • Meets all required competencies for department, unit and/or hospital.
  • Responsible for performing all other duties as assigned.
Qualifications
  • Work requires the basic knowledge of theories, principles, & concepts acquired through the completion of an Associate degree in Computer Science, 
  • Information Systems or 2 years of healthcare experience. 
  • Requires proficiency with highly technical clerical tasks, which are specific to an area of work at the expert level. 
  • Experience with an EMR preferred.  Experience working in a provider’s office preferred.

Pay transparency:  St. Joseph’s Health provides a salary range to comply with New Jersey Law.  The rate of pay for each position will be determined based on a variety of factors including the candidate's relevant experience, qualifications, skills, etc.” The salary range does not include incentives, differential pay or other forms of compensation.

St. Joseph’s University Medical Center is an academic tertiary care medical center and state designated trauma center, located on the Paterson campus, regularly accepts referrals of difficult or unusual cases from other hospitals and physicians and performs both complex and routine procedures.

Skills Required
