Catastrophic Case Manager

Your goal is to provide for quality, cost-effective outcomes for the client through collaboration and communication with the client, the family, the physician, and other members of the healthcare team.

Responsibilities:

·         Process the case utilizing care management model and tools

·         Develop a plan of care plan through interdisciplinary collaboration, which identifies realistic short-term and long-term goals

·         Identify, secure, and coordinate the services and resources to implement the plan

·         Monitor services being provided for continued appropriateness

·         Evaluate the clients progress, the plan, and the appropriateness of the services provided, with modifications as necessary

·         Promote the client’s/family’s self-advocacy skills so achieve maximum self-sufficiency

Requirements:

·         Active CA Registered Nurse License

·         Bachelors of Science degree in Nursing preferred

·         CCM, CRRN, CDMS, or CRC preferred

·        2 years minimum related experience in catastrophic case management with worker’s compensation

·         Basic knowledge of computer and Microsoft Office products

·         Valid driver’s license and liability insurance

·         Demonstrate effective, accurate, and timely verbal and written communication in all work situations

·         Ability to organize and evaluate workload and prioritize projects to ensure deadlines are met

·         Detail oriented with the ability to complete multiple, simultaneous projects with accuracy and efficacy

·         the ability to travel to on-site medical appointments
