Overview:

The Registered Nurse provides case management and nursing services to coordinate all aspects of the care of individual. The primary goals of the RN is to optimize the functioning of recipients who have complex medical needs along with other challenges by coordinating the provision of quality treatment and care for psychiatric and medical, and support services in the most efficient and effective manner. The RN case manager is responsible to coordinate/oversee medications and monitor/track the condition of the progress in conjunction with the clients and their treating ARNP/MD and/or primary care provider and other providers to ensure the appropriate care is provided. 

ESSENTIAL/CORE JOB-SPECIFIC DUTIES AND RESPONSIBILITIES  

1. Assessment:  Conducts comprehensive nursing assessments that considers the relationship between mental health and physical health. Screen and address client medical problems/risk factors and obstacles to necessary care.  Updates required assessments in accordance with Client Record Handbook guidelines. Finalizes all documents in Avatar and turn in all assessment within 48 hours. 
2. Treatment Plan:  Develops individualized service plans that include measurable goals and objectives in partnership with the youth and their families. Complete service plans and reviews that address both youth and family factors. Assist with the implementation of the service plan with emphasis on helping family members achieve autonomy in meeting youth and family needs. Updates service plans at least every 3 months or at any time significant changes occur in the client’s condition, situation, or circumstances (e.g., hospitalizations, achievement of goals, etc).
3. Nursing Services:  Implement psycho-pharmacologic and medical treatment and medication policies and procedures, including transcribing, administering, evaluating, and recording psychotropic medications prescribed by the team psychiatrist/ARNP in accordance with Agency policy/unit procedures (i.e. “read back” process).  Manages pharmaceuticals and medical supplies, organizes and manages the system of getting medication to persons served. Provides assistance and support to youth and families on an individual, group, and family basis in the office and in community settings to teach techniques to alleviate and manage symptoms
4. Patient Care: The Registered Nurse performs patient care, including but not limited to: patient assessment; medication administration; monitor patients for medical problems and medication side effects; plans care; provides crisis intervention; functions as a client advocate and liaison; makes appropriate referrals and consultations both within and outside the agency; completes and processes all required documentation; engages in health promotion, prevention, and education activities with patients and staff; and participates in team building and quality improvement activities. The position demonstrates the professional skills and behaviors to promote comprehensive, and meaningful healthcare for consumers in a diverse healthcare environment.
5. Progress Notes:  Documents all services provided in the client record in accordance with Client Record Handbook Guidelines.  Documentation must be finalized in Avatar within 48 hours from the time the service was rendered. Progress notes must be detailed and clearly reflect how the nurses efforts are linked to the services and goals in the client’s service plan and the client’s progress or lack of progress relative to the service plan.

Education/Experience: 

• Previous psychiatric nursing experience preferred.
• Experience and/or wiliness to work in off-site community settings preferred.
• Experience working with youth and family system preferred.

Licensure or Certification:

• Licensure as a registered nurse in accordance with Florida Statute 464.
• Must maintain current CPR and CPI certifications.
• Must possess current, valid driver’s license and maintain Apalachee Designated Auto Driver status.
