GENERAL FUNCTION:
Under the direction of the Owner, Executive Director and Program Administrator, the Case Specialist is responsible for supervising ILS/SLS/AST Educators in the implementation of Individual Service Plans (ISP) through the guidance of area Regional Centers, focusing on increasing consumer independence by providing independent living skills to adults with developmental disabilities. Case Specialist will support consumers as they receive services which supports their right to choice, independence, and self-empowerment while establishing realistic goal opportunities in the areas of self-care, public transportation, socialization and vocational exploration.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Work collaboratively with ILS/SLS/AST Educators and Field Supervisors to identify and evaluate the development of specific and realistic consumer goals related to teaching independent living skills.
- Preparing and implementing quality assurance policies and procedures and case management
- Performing routine calls a minimum of 1x per month to active consumers, calling consumers daily to ensure quality of service.
- Communicate with appropriate department at Regional Center and provide updates on consumer progress.
- Call consumers that are on hold to identify the status of their services.
- Identify and ensure all active consumers have active authorizations.
- Communicate with Intake & Scheduling Department regularly.
- Update internal documentation platforms.
- Work collaboratively with scheduling department to assign educators to new or existing consumer's and encourage their active involvement in proper decision-making, problem-solving and conflict resolution.
- Schedule educators for quarterly written evaluations of consumer's development and behavioral interaction; review reports for accuracy and proper documentation.
- Conduct functional assessments to in-coming consumers to establish realistic treatment programs that effect the development of specific goals and objectives.
- Submit Special Incident Reports within 24 hours, per requirement from Regional Center.
- Complete Quality Assurance calls with employee’s mid-month to ensure authorized hours are being completed.
- Maintain active communication and provide feedback to ILS/SLS/AST Educators, Field Supervisors, and RC Service Coordinator; represent company during IPP meetings and consumer assessments.
- Advocate and work closely with consumer through governmental agencies such as Medi-Cal, AFDC, SSI, IHSS and other non-profit organizations.
- Identifying and resolving workflow and service issues.
- Ensure authorized hours are being met each month
- Addressing and discussing issues and proposed solutions with Executive Department.
- Documenting quality assurance activities.
- Making recommendations for improvement.
- Creating training materials, as needed.
KNOWLEDGE AND REQUIREMENTS:
- Ability to be creative, proactive and sensitive in solving consumer's problem and complex issues
- Valid CA Driver's License & Good driving record-Reliable Auto & Valid Insurance
- Cell Phone with Reliable Service (For Communication)
- Able to Work Independently; Organized / Professional / Responsible; Energetic Personality / Self Motivated
- Valid CPR and First Aid certification
- Ability to take all educational training by Roman Empire
- Criminal Record clearance, child abuse index report, TB clearance
EDUCATION:
BA/BS preferred
3 years' supervisory experience in the field of social services and case management or related field preferred
Skills Required
