As a Case Manager I, you’ll provide hands-on support to veterans as they work toward stable housing, employment, and overall well-being. You’ll work under the guidance of experienced supervisors and be part of a team that values collaboration, mentoring, and professional development.
What You’ll Do

Support Veterans One-on-One: Help veterans identify and work toward personal goals in housing, health, employment, education, and recovery.

Coordinate Care: Connect clients with VA services and community partners to ensure access to mental health, physical health, legal support, and more.

Develop Individual Action Plans (IAPs): Collaborate with clients and your supervisor to create clear, goal-oriented plans.

Track Progress: Maintain accurate client records and progress notes, and update plans as needed.

Facilitate Life Skills Support: Assist in group classes and workshops focused on daily living, wellness, and goal achievement.

Conduct Home Visits: Check in on clients in their housing environments to provide ongoing case management and support.

Transport Clients in Agency Vehicles: Assist clients with getting to appointments, interviews, court appearances and other meetings as needed.

Collaborate & Learn: Attend regular team meetings and trainings to stay connected and build your skillset.
Programs You May Support
This role may be assigned to one or more of the following programs:

Transitional Housing: Help veterans prepare for and transition into permanent housing.

SSVF (Supportive Services for Veteran Families): Deliver rapid rehousing and homelessness prevention services.

Permanent Housing (HUD-VASH, etc.): Focus on housing stabilization and long-term support.

Workforce Development: Assist clients with employment plans, resumes, and job readiness.

Justice-Involved Veterans: Provide pre-release and post-release support for veterans exiting incarceration.

Aftercare and Counseling Services: Offer ongoing case management and facilitate group or individual therapy (if licensed or certified).
- High School Diploma required.
- One year of experience in social services or related field preferred. Case Manager I employees with over five years of experience in social services may have additional job duties as outlined in Case Manager II job description and may require less supervision.
- Experience working with homeless and/or veterans preferred.
- Certification in Substance Abuse Counseling preferred.
- Experience facilitating support groups preferred.
- Strong oral and written communication skills.
- Professional manner and appearance.
- Ability to work independently and within a team.
- Ability to take direction.
- Ability to work effectively with diverse group of clients, staff, and community members.
- Computer proficient in Microsoft Office and Internet.
- Valid drivers license required. Must meet company insurance requirements and complete a provided driver training course.
