Days/Hours:               Varies Monday – Friday 9:00am – 5:00pm, some evenings 11:30am -7:30pm, subject to change depending on program needs.

The organization has evolved into one of the nation’s preeminent reentry and justice-informed service organizations, providing formerly incarcerated people with the skills and wrap-around services needed to break the cycle of crime and incarceration and to build productive lives in their communities. 

Fortune has grown steadily over the years to an agency with close to $90 million in annual budget with just under 600 staff. We anticipate that we will keep growing, both in size and depth of service. Fortune has a dual mission: both advocacy and service. The advocacy portion of our mission focuses on building a more just criminal legal system and reducing the barriers to reentry into society. The service side of Fortune has evolved to encompass a broad range of programs that started with employment and education and now includes a robust array of services, among them: alternatives to incarceration, supervised release, court advocacy, discharge planning within correctional facilities, licensed substance use and mental health treatment, a housing continuum ranging from emergency and transitional through permanent congregate and scattered site housing, assistance in obtaining benefits, a care management unit, HIV services, a food and nutrition program, and an arts program. Each service we provide is informed by the needs of the population we serve. Fortune currently serves approximately 14,000 people a year. Our program models are recognized, both nationally and internationally, for their quality and innovation. We have locations in Brooklyn, The Bronx, Manhattan, and Queens.


Position Summary:

The Case Manager is responsible for completing client intakes for the unit, managing a caseload, and providing group facilitation when needed. Case management duties include individual sessions, developing treatment plans, documenting client progress for court reporting, and making referrals to internal and external services. The Case Manager will report to the unit’s Program Director.

In addition to traditional case management responsibilities, this role includes duties aligned with Fortune Society’s Mentor/Credible Messenger model. The Case Manager will serve as a positive role model, leveraging lived experience and community credibility to build trust, foster accountability, and support participants in achieving personal and legal goals.

Essential Duties and Responsibilities:

  • Conduct intake for new clients enrolling into the program;
  • Perform general case management duties, including weekly individual sessions with clients, referrals to community-based services, developing client schedules, and treatment plans;
  • Assist clients in identifying and prioritizing needs;
  • Provide referrals to ensure all participants’ concrete needs are addressed;
  • Ensure accurate and timely data entry of all client information into the agency’s database;
  • Provide accurate and timely information in court reports;
  • Monitor participants’ attendance and compliance with mandate requirements;
  • Conduct and track client’s toxicology;
  • Participate in case conferences for optimum collaboration among departments/programs and external agencies;
  • Attend and participate in meetings, conferences, and training;
  • Conduct group facilitation services when needed;
  • Serve as a Mentor/Credible Messenger by sharing relevant lived experiences to build rapport and trust with participants;
  • Model pro-social behavior and provide guidance on conflict resolution, accountability, and personal development;
  • Engage in outreach and follow-up to maintain consistent engagement with participants, especially during critical transition periods;
  • Engage in his/her own ongoing professional development;
  • Perform other duties as assigned.




Qualifications

Qualifications:

  • One year of case management experience preferred;
  • High school diploma or GED/HSE required;
  • Experience and commitment to working with mandated participants with criminal justice involvement;
  • Strong Time Management Skills;
  • Strong Microsoft Office and database entry skills;
  • Strong assessment and engagement skills;
  • Strong written and verbal communication skills;
  • Facilitating group sessions;
  • Bilingual English/Spanish a plus;
  • Strong organizational and leadership skills;
  • Lived experience with the criminal justice system and/or history of community engagement is strongly valued;
  • Ability to connect authentically with participants and foster a supportive, nonjudgmental environment.
