Summary/Objective: Responsibilities include reviewing emails, checking phone messages, following up on referrals, reviewing documentation, ensuring proper paperwork and consents are completed and signed, conducting discharge planning, conducting utilization review, working well with other case managers, demonstrating strong communication skills, and working with a team.
Essential Functions
Maintain confidentiality about patient information
Ability to interact with patients, co-workers, medical practitioners, and superiors in a professional manner
Proficiency in reading medical records/charts
Works well with a variety of age groups

3 years of experience as an RN

Previous Case Management Experience
Use of the Epic EHR system

What We Do
Southern Humboldt Community Healthcare District is a family of services that includes Jerold Phelps Community Hospital, with a 24-hour emergency room, acute care inpatient unit, lab, radiology, mammography, and an 8-bed skilled nursing facility; Southern Humboldt Community Clinic; Southern Humboldt Family Resource Center; and Garberville Pharmacy.
