Responsible for the day-to-day decisions regarding coordination of care, utilization management, and discharge planning. Manages patient caseloads coordinating plan of care and discharge. Maintains full responsibility ensuring customer and physician satisfaction is achieved.
JOB SPECIFICATIONS AND CORE COMPETENCIES
Education: Knowledge of clinical practice and case management processes normally acquired by completing a Bachelor’s Degree in Nursing with additional training/certification in case management. BSN preferred.
Licensure: RN Indiana license. Certification in an area of case management is preferred.
Experience: Three to five years of clinical nursing experience with demonstrated leadership and management abilities. Critical Care experience is highly desirable.
Other Job Requirements: Demonstrates critical thinking skills. Demonstrates clear, effective communication to include verbal, written, and listening skills.
Demonstrates competency in computer applications.
May require the ability to speak Spanish.
