Position Purpose

As a Case Manager, you will play a vital role in helping individuals and families navigate complex challenges by providing client-centered support and connecting them to essential services. This position is ideal for someone passionate about social justice, advocacy, and holistic care coordination.
 

What You Will Do
 

As a Case Manager, you will:

  • Conduct comprehensive assessments to identify client needs, strengths, and barriers.

  • Develop individualized service plans with achievable short- and long-term goals.

  • Connect clients to housing, healthcare, employment, behavioral health, and other community resources.

  • Monitor progress through regular follow-ups and adjust care plans as needed.

  • Maintain accurate documentation in compliance with HIPAA and organizational standards.

  • Collaborate with internal teams and external partners to ensure continuity of care.

  • Advocate for clients to access and retain benefits.

  • Facilitate crisis interventions, discharge planning, and case conferencing.

  • Participate in team meetings, supervision, and ongoing professional development.
     

Minimum Qualifications

  • Education:

    • Required: Bachelor’s degree in Social Work, Human Services, Psychology, Public Health, or related field.

    • Preferred: Master’s degree in a related field.

  • Certifications:

    • Required: CPR/First Aid

    • Preferred: Certified Case Manager (CCM) or equivalent.

  • Experience:

    • Required: At least 1 year in case management or social services.

    • Preferred: 2–3 years working with underserved populations such as the homeless, elderly, or those with chronic health conditions.
