Job Details:The School of Medicine is searching for an Case Manager who resides in the 10 county South Georgia Healthy Start Service area (Appling, Bulloch, Candler, Emanuel, Evans, Jefferson, Jenkins, Screven, Tattnall, Toombs).


 

Responsibilities:

Case Managers will assist with the execution of South Georgia Healthy Start's (SGHS) daily activities to support women, infants, and families in rural South Georgia.  Case Managers will be responsible for recruiting, enrolling, assessing, and retaining participants; providing care navigation and social services enrollment assistance; performing routine check-ins with participants; delivering evidence-based curricula and health education; conducting parenting education classes;  and actively engaging community partners within the SGHS assigned service region.  

Case Managers will be expected to participate in professional development and trainings as applicable and may be assigned to lead committees or initiatives within the SGHS program.

Qualifications:

Bachelor’s and two years of relevant experience, which could include case management, patient navigation, health education, community engagement, survey research, interventionist work, or doula/birthworker experience. In lieu of a bachelor's degree, an equivalent combination of education and experience will be considered. 

Valid current driver's license is required.

Knowledge/Skills/Abilities:

  • Exceptional interpersonal skills.

  • Strong verbal and written communication skills.

  • Ability to learn and utilize cloud-based case management systems.

  • Ability to travel regularly within the 10-county SGHS project area, as well as to travel occasionally to meetings, trainings, and events throughout the state and in other states.
