Your Opportunity

At Mission Australia, the Camperdown Support Services (CSS) team delivers a holistic, person centred support model for residents of Common Ground Sydney, including individuals with lived experience of homelessness who have successfully transitioned into permanent housing. Working within a unique integrated housing community, you will play a vital role in empowering residents to thrive. In this rewarding on-site position, you will provide strengths-based case management, helping residents build the skills and confidence needed to sustain their tenancies, enhance their daily living capabilities, and achieve greater independence, stability, and long-term wellbeing.

Working Mission Australia Housing and a broad network of health, community, and welfare providers, you will develop tailored support plans, advocate for residents, coordinate services, and help individuals navigate complex challenges including homelessness, mental health concerns, substance use, and social isolation. Using a strength based and trauma informed approach, you will build trusted relationships that empower residents to achieve sustainable outcomes and thrive within their community.

Your Key Responsibilities:

  • Deliver comprehensive case management services to residents, developing, implementing, and reviewing individual support plans aligned to their goals and needs.
  • Conduct intake assessments, risk assessments, support reviews, and ongoing case evaluations to ensure appropriate interventions and positive outcomes.
  • Support residents to successfully sustain their tenancy by building independent living skills, enhancing wellbeing, and strengthening community connections.
  • Advocate for residents and assist them to access appropriate housing, health, mental health, community, financial, and welfare services.
  • Coordinate internal and external service providers to ensure residents receive holistic, integrated, and consistent support.
  • Provide practical assistance and crisis support, including facilitating access to resources, emergency relief, vouchers, and other relevant supports.
  • Maintain accurate case notes, client records, reports, and administrative documentation in accordance with organisational and legislative requirements.

Requirements for Success

  • Relevant degree with at least one year's experience, an Associate Diploma with relevant experience, or a lesser qualification supported by substantial relevant experience in community services or a related field.
  • Demonstrated case management experience supporting people experiencing homelessness, mental health challenges, alcohol and other drug issues, family and domestic violence, or other complex support needs.
  • Strong understanding of trauma informed, strengths based, and person centred practice, with the ability to support clients through complex life circumstances.
  • Excellent relationship building and client engagement skills, with the ability to establish trust while maintaining appropriate professional boundaries.
  • Proven ability to remain calm, resilient, and professional when responding to challenging behaviours, crisis situations, conflict, and de-escalation requirements.
  • Strong advocacy, communication, and stakeholder engagement skills, with the ability to work collaboratively across multidisciplinary teams and external service providers.
  • Sound organisational and administrative skills, including experience maintaining accurate documentation, managing competing priorities, and using client management systems effectively
  • First Aid certificate is desired but not mandatory.
  • Full driver licence with no restrictions.
