JOB SUMMARY: Responsible for the implementation of the Case Management System. The Case Managers work with the Interdisciplinary Team to ensure that all people supported are reaching their goals. Ensure that all documentation is completed and filed according to Opportunity Works Policy and all Licensing and Accreditation Standards. Provide advocacy and support to individuals served in the ABI, CBDS and/or Day Habilitation.
QualificationsQualifications:
- Bachelor’s Degree preferred, minimum High School Diploma or equivalent
- Experience with Case Management and Case Reviews in a relevant health care setting
- This position requires excellent organizational skills as well as excellent verbal and written communication skills.
- Valid driver’s license and the ability to drive a van preferred
- Crisis intervention training (i.e. PABC) is favorable
- CPR, First Aid and MAP certificates preferred
