SUMMARY:

Family Service League is seeking a full-time Case Manager for our Health Home program. The Case Manager meets the intensive needs of the population while meeting visit requirements as stipulated by the DOH for the Health Homes program. Clients include persons with behavioral health, medical, and/or substance abuse diagnoses.

RESPONSIBILITIES:

  • Maintain a positive, working rapport with Suffolk County Adult SPOA.
  • Employ a culturally sensitive and proactive outreach approach to potential members.
  • Manage a caseload of 40-50 clients and meet their intensive needs.
  • Meet minimum visit requirements based on client's determined acuity level and needs.
  • Maintain the EHR and ensure relevant clinical documentation is received from the referral source and uploaded into the EHR to serve as eligibility documentation.
  • Speak with mental health, medical, and substance abuse providers in addition to hospitals, jails, and housing/shelter providers if necessary.
  • Form and maintain a positive and professional relationship with both internal and external providers to coordinate care and obtain needed information.
  • Act as a professional role model with staff and inspire them to achieve FSL mission and program goals.
  • Complete all required documentation in a timely and accurate manner.
  • Report all incidents to the program management staff.
  • Follow all agency and program mandates as set by FSL, Department of Health, and Suffolk County Division of Mental Hygiene.
  • All other duties as assigned.


QUALIFICATIONS:

A bachelor’s degree in a health-related field is required.

Minimum of two years’ experience providing direct services to people with Serious Mental Illness, medical conditions, developmental disabilities, and/or substance abuse preferred.

Proficiency in Microsoft Office required; experience with an EHR is preferred.

Strong assessment, engagement, organizational and time management skills.

Excellent verbal and written communication skills are required.

Valid, clean New York State Driver’s License required.


PHYSICAL REQUIREMENTS:

This position often requires sitting and working at a computer for extended periods of time, traveling to different FSL offices, community meetings, home visits, etc.
