As a member of a multidisciplinary team, the Time Limited Rental Assistance Program (TLRA) Case Manager provides comprehensive counseling, case management, advocacy, and group work services to young adults ages 18–24 who are experiencing homelessness. The Case Manager supports participants as they transition into permanent housing while building employment readiness, educational attainment, and essential independent living skills. This role is rooted in trauma-informed, strength-based, and youth development practices.

Key Responsibilities Direct Service & Case Management
  • Oversee the case management needs of young adults experiencing homelessness funded through HUD.
  • Provide direct services, including individual counseling, to participants at least twice per month. This includes conducting home visits.
  • Develop and monitor individualized service plans and action steps in collaboration with participants.
  • Utilize evidence-based tools to assess housing history, homelessness risks, daily functioning, socialization, health, and wellness.
  • Assist participants in managing and resolving potential crises.
  • Provide quarterly aftercare services to participants who successfully complete the program.
Advocacy, Referrals & Skill Building
  • Identify and connect participants to resources that support independent living.
  • Provide advocacy and referrals for entitlements, legal assistance, education, vocational training, medical, and mental health services.
  • Coach participants in developing life skills including budgeting, health management, recreation, and social development.
  • Escort participants to appointments as needed.
Collaboration & Documentation
  • Participate in intake, referral, interviewing, and orientation processes.
  • Maintain accurate and timely documentation in the GSS database and all required city and funder systems.
  • Collaborate with internal staff and external service providers.
  • Represent the program and organization at external meetings, trainings, and community forums.
  • Participate in required meetings, supervision, and professional development activities.

QualificationsRequired
  • Bachelor’s degree required.
  • Minimum of two (2) years of experience providing services to at-risk youth and young adults, including LGBTQ+ youth.
  • Experience conducting assessments, providing individual and group counseling, and implementing individualized service plans.
  • Demonstrated collaboration with community-based organizations and NYC systems.
  • Understanding of the needs of young adults experiencing homelessness.
  • Commitment to strength-based, youth development, and trauma-informed practices.
  • Cultural humility and sensitivity to diverse racial, ethnic, and LGBTQ+ communities.
  • Strong crisis intervention, advocacy, and problem-solving skills.
  • Excellent written and verbal communication skills.
  • Flexibility in work schedule to ensure participant access and availability.

Skills Required
