Community-Based | Serving Homeless & Mental Health Populations
Position SummaryWe are seeking a compassionate and driven Case Manager to provide community-based support to individuals experiencing homelessness and behavioral health challenges. This role focuses on connecting participants to critical resources, supporting recovery goals, and advocating for services that promote independence and stability.
The Case Manager works as part of a multidisciplinary team to deliver person-centered, recovery-oriented care while demonstrating a strong commitment to APIS and RHD values.
Key ResponsibilitiesTeam-Based PlanningCollaborate with a multidisciplinary team to support participants from a holistic, values-based perspective
Assist in developing and updating individualized recovery plans
Partner with clinical, medical, and community stakeholders to ensure coordinated care
Build and maintain relationships with community agencies and service providers
Provide community-based services, including:
Resource connection
Life skills support
Community integration
Support participants in achieving recovery plan goals
Transport participants to appointments and services as needed
Deliver services within the program’s community service area
Complete all required:
Assessments
Documentation
Progress notes
Ensure compliance with:
Organizational policies
Funder requirements
Licensing and accreditation standards
Perform additional duties as assigned
Bachelor’s degree in social science related coursework or related field preferred
A high school diploma and 12 semester credit hours in social sciences and at least two years direct care experience in behavioral health, addiction, IDD services or high school diploma and at least five years of direct care experience in behavioral health, addiction, or IDD services
Registered nurse
2 years of experience supporting individuals in behavioral health, addiction recovery, or IDD services- experience with homelessness, behavioral health, addiction, IDD services preferred
1 year of case management experience preferred
Valid driver’s license and reliable transportation required
Strong communication and interpersonal skills
Ability to build trust with diverse populations
Knowledge of community resources and service coordination
Crisis intervention and problem-solving skills
High level of organization and attention to detail
Commitment to ethical practice, confidentiality, and professionalism
Cultural humility and ability to work with diverse populations
Ability to work independently and as part of a team
Basic computer proficiency (email, documentation systems, data entry)
Knowledge of behavioral health and/or residential care regulations preferred
CPR/First Aid certification (or willingness to obtain)
Must pass required background screenings (including FBI clearance, if applicable)

What We Do
South Jersey Behavioral Health Resources, Inc. has been providing behavioral health services to adults, children and families for over 25 years. The services provided include, Residential, Outpatient (OP), Intensive Outpatient (IOTSS), Adult Partial Care (APC), and Homeless Services.
