Education/Experience:
Applicants must meet any one of the following education and experience requirements:
- HS degree with six (6) years of direct service experience in the Mental Health field, OR
- Associates degree with four (4) years of direct service experience in the Mental Health field, OR
- Bachelor’s degree with two (2) years of experience working in the mental health field.
License or Certification:
Valid California Driver’s License and current automobile insurance and must maintain a driving record that meets the company driver's eligibility policy.
NOTE: This position requires driving from home to home, working with clients in the community, and transporting clients in personal vehicles.
How you will make a difference (job overview):
This position has primary responsibility for providing oversight and care management to the clients referred to Starlight programs. The Case Manager will provide services that may include case management/linkage, assessments, screenings, treatment planning/monitoring, and rehabilitation services to the clients and their families.
Division/Program Overview:
- Focus on providing prevention and early intervention to individuals (10 to 25) who are at risk for experiencing early psychosis.
- Working as a team to provide quality and comprehensive care to individuals and their families.
- Sessions are in the community, clinic, home and some telehealth.
- We provide services to individuals and families throughout Santa Clara County.
