ESSENTIAL JOB DUTIES AND RESPONSIBILITIES

Responsibility for the provision of in-school prevention and case management services to assigned caseload.  Services include assessments, service plan development and group work services.  Case management services, which include assessment of needs, referrals, linkage, coordination and monitoring.

Works collaboratively with School Administration, Personnel and Program Supervisor as point of access while following all case Opening/Closing procedures.

Participates as a member of an interdisciplinary team that includes the Program Supervisor, Student, Family, Teacher and other professionals. 

Provides crisis intervention, when necessary, to stabilize students and their family to support and encourage maximum school performance, attendance, and achievement by alleviating barriers to learning.

Advocates for student/family interest and makes appropriate referrals to human service providers in the community to meet the identified needs.

Attends staff meetings at assigned school(s) and School Based Services Team Meetings.  Daily activities require coordination and approval by the School Administration.  
Reports to School Administrator or designee and Program Supervisor/Director daily. 

Performs classroom observations and individual observations as assigned. In conjunction, provide follow-up and consultation as needed. Assists in writing behavioral plans for classroom and at home.

Co-facilitates, with a Master’s Level Clinician, provide a minimum of two (2) Professional Development opportunities, and two (2) Parent Education opportunities, per school, per fiscal year.

Documents services delivered in a timely, accurate and prescribed manner in compliance with the Agency’s quality assurance plan.  

Meets direct service standards as defined by the Agency administration.

Deliver prevention-based services and community mental health screenings to children, youth, and families, guided by Strength and Difficulties Questionnaire (SDQ) outcomes.

Utilize the Michigan Model for Health curriculum to deliver education and prevention services.

Develop and implement a referral process within the school(s) for:
•    School-based prevention services
•    Collaboration with school-based health clinicians

    Keeps up to date with current trends in service delivery systems

    Participate in administrative staff meetings, agency committees, and other projects, as assigned.

    Adheres to the MiSide policies and procedures.

    Adheres to the MiSide code of ethics, and complies with local, state and federal laws.

    Any additional task and/or duties, as assigned.

Qualifications

EDUCATION & EXPERIENCE
Bachelor's degree (BSW/BA) in social work, psychology, or a degree in a related field, required
•    One-year experience and training in examination, evaluation and treatment of minors and their families
•    Fluency in Spanish, a preference, for specific school locations.

REQUIRED LICENSURE AND CERTIFICATIONS
•    Appropriate professional registration/certification by state/national authority required (LLBSW, LBSW).
•    Possession of a valid driver’s license and have access to a private vehicle for day-to-day job performance
•    Understanding developmental stages, age-appropriate behaviors, and common challenges facing school-age youth. 
•    Ability to identify at-risk students, conduct needs assessments and recognize warning signs.
•    Knowledge of Evidenced-based prevention models, risk and protective factors, and early-intervention strategies. 
 

Skills Required
