Job Description:

Case management services are designed to assist patient and family members with continuing care arrangements that may be necessary following discharge from the hospital. Case managers work with physicians, nurses, physical therapists and other members of the health care team to identify and coordinate alternative care or services that will assist in recovery. 

 

The Case Manager strives for excellence in direct patient care by building a collaborative caring work environment coordinating care and information across the various disciplines focused on patient outcomes and wellness. The Case Manager assesses plans and facilitates options and services which best meet the patients’ healthcare needs in the continuum of care. The Case Manager formulates goals with the patient and their families. The Case Manager advocates for the patient through communication and evaluation of available resources to promote quality cost-effective and safe outcomes. The Case Manager incorporates standards of care for Case Management including identification/selection of patients for case management services, problem identification, planning, monitoring, evaluating, transition of care and outcomes. The Case Manager implements practice based on current research and evidence based practice and ensures adherence to hospital and department policies, regulatory standards, and guidelines. The Case Manager supports an environment of caring focused on the patient’s healing and wellness.

Job Description: 

  • Identifies and makes appropriate referrals for quality issues to promote optional outcomes.  

  • Identifies and makes appropriate risk management referrals and provides for patient safety.  

  • Supports, role models, and educates patient and family on recommended infection control practices.  

  • Documents in Case Management systems and the medical record. 

  • Assesses appropriateness of admission and determines correct patient status using payer criteria and  

  • clinical assessment.  

  • Screens all patients to identify the need for case management services.  

  • Gathers relevant, comprehensive information and data required for assessment and obtains information by interviewing the patient/family, the physician, other members of the healthcare team and appropriate individuals.  

  • Prioritizes needs of patient/family based on patient’s goals.  

  • Evaluates objectively and critically all information related to the current or proposed treatment plan to  

  • identify potential barriers, clarify or determine realistic goals and objectives, and seek appropriate alternatives for the patient.  

  • Ensure the patient’s involvement in assessment and planning and provides and encourages opportunities for the patient to communicate and collaborate with the case manager and the healthcare team. 

  • Engages the patient/family in the planning process as the primary decision maker and goal setter and communicates directly with the patient/family in order to understand and incorporate priorities of the patient d) Educates and prepares the patient/family to make informed and appropriate decisions or provides educational resources. 

Job Requirements: 

  • Bachelor's Degree in Nursing 

  • Ky RN license 

  • Five years direct clinical experience in an acute care hospital 

 

Preferred Requirement: 

  • Telemetry experience
