JOB DESCRIPTION

Summary/ Objective

East Side House (ESH) is a community resource in the Bronx and Northern Manhattan. We believe education is the key that enables all people to create economic and civic opportunities for themselves, their families, and their community.  ESH’s Adult & Workforce Development Programs aligns with our mission by providing high-quality programs and services within community centers and school partnerships.  

 

Reporting to the Employment Coordinator, the Case Manager is responsible for a key role in the PSP staff inter-disciplinary team whose purpose is to provide comprehensive educational and emotional support that enable young adults to meet their personal, educational and employment goals.  The Case Manager will support PSP participants in choosing a post-secondary track and completing outlined track deliverables for certification training, professional soft skills, internship hours, and job placement.  As a member of the Adult & Workforce Development Programs, the Case Manager is expected to communicate effectively with the Assistant Director of PSP and Employment Coordinator to facilitate participant intake, case management and akll other wrap around services needed for participants who are enrolled in GED/ALE, PSP Academy, CRED and various other PSP service areas.  This position’s direct reports include INDICATE WHO IS UNDER THE POSITION’S SUPERVISION and interacts with INDICATE WHICH OTHER DEPARTMENTS.  With a focus on INDICATE WHAT AREA, the NAME POSITION provides INDICATE WHAT SERVICES/ OUTCOMES.   

 

 

 

Essential Functions and Responsibilities

Under the supervision of the Employment Coordinator, the Case Manager, will be responsible for but not limited to the following:

  • Caseload & Engagement: Manage a caseload of approximately 50–100 participants per year (organized by cohorts); maintain regular contact at all phases to support progress, engagement, and retention.  

 

  • Intake & Planning: Conduct participant intake, needs/barrier assessments, and develop individualized service or goal plans; provide regular follow-ups and adjust plans based on progress and emerging needs. 
  • Attendance & Retention: Conduct daily/ongoing outreach regarding attendance and engagement; use proactive strategies to reduce barriers and improve persistence.  
  • Interdisciplinary Coordination: Collaborate with Instructors, Retention Specialist, and Education/Employment Services Managers to deliver coordinated, co-located services and ensure participant goals are met. 
  • Counseling & Coaching: Provide one-on-one and small group counseling/coaching that supports academic, career, and personal goals; deliver timely feedback on progress and next steps. 
  • Workshops: Facilitate workforce readiness and basic life skills workshops aligned to participant needs and program objectives. Complete necessary training for instructor credentials in CPR-First Aid and other competencies. 
  • Referrals & Navigation: Connect participants to services across the local workforce development system (e.g., Career Centers, NYS Department of Labor, ACCESVR, NYS Commission for the Blind, TANF funded supports) and to community resources addressing housing, food security, legal aid, and benefits.  
  • Recruitment & Onboarding: Support recruitment and interviewing for new cohorts; maintain accurate participant files and records in accordance with funder and organizational standards. 
  • Data & Compliance: Build and maintain a shared database (e.g., Google Sheets/Drive/Salesforce) for caseload tracking; enter thorough, timely case notes; prepare reports and participate in data driven case conferences. 
  • Meetings & Development: Participate in team/department meetings, training courses, case conferences, workshops, and on/offsite events; contribute to continuous improvement of PSP services.

Education / Experience 

  • Bachelor’s degree in social work or a related field and experience working with City systems (e.g., justice, public assistance, child welfare, education, housing); or 
  • Associate’s degree and at least three (3) years of successful experience providing case management/counseling and working with City systems.

Expected Work Schedule
The work schedule expected for this position is: Monday through Friday, 9:00 a.m. to 5:00 p.m and possible weekend events, as aligned to the needs of the program/ service.   The schedule may be adjusted based on ESH’s operational needs, with notice provided whenever possible.
