Case Manager will provide direct support and case management services to senior residents, promote their health and well-being, and help foster a safe, supportive, and thriving community environment. Purpose of Position: The purpose of the Case Manager position will be to join a team of social workers to provide social services that enhance the clients’ overall quality of life and increase their access to a variety of resources. Our focus is on financial stability, health and wellness, food, and housing security. The ideal candidate will possess strong interpersonal skills to facilitate collaboration with the team and together deliver impactful results
 

Schedule: Monday through Friday 35 hours per week


Salary Range: $47,000-$50,000


Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate’s number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.

The Case Manager is committed to supporting the dignity, independence, and well-being of older adults by providing quality services and creating opportunities for residents to thrive in a healthy, supportive, and inclusive environment.
 

Role, Responsibilities, and Essential Duties:

  • Provide comprehensive case management and social service support to senior residents.
  • Assist residents with navigating HRA systems and applying for public benefits, including SNAP (Food Stamps), Medicaid, and benefit recertifications.
  • Connect residents with community resources and outside service providers based on individual needs.
  • Provide basic technical assistance to seniors, including support with phones, tablets, computers, and online applications.
  • Develop, coordinate, and facilitate educational, recreational, and wellness groups and activities that promote healthy aging and social engagement.
  • Conduct assessments and maintain accurate case notes and documentation in accordance with agency policies and procedures.
  • Advocate on behalf of residents and collaborate with internal staff and external partners to ensure residents receive appropriate services.
  • Support residents in maintaining independence and improving their overall quality of life.
  • Participate in team meetings, trainings, and other program-related activities.
  • Maintain confidentiality and uphold professional standards and ethical practices.
  • Work with other staff to assure charts are up to stander and guideline within our program.
  • Accompany client to outside agencies if needed.
  • Meet with new residents and support in creating intake form.

Qualifications/Educational Requirements:

  • Bachelor's degree required; Preferred degree in phycology, human resources, arts, social work, and/or sociology.
  • Minimum of one year of experience in case management, social services, or working with older adults and leading groups
  • Knowledge of HRA benefits, SNAP, Medicaid, and other public assistance programs.
  • Strong organizational, communication, and interpersonal skills.
  • Bilingual (Spanish or any other languages)
  • Ability to work effectively with diverse populations and demonstrate empathy and cultural sensitivity.
  • Experience facilitating groups and developing community-based programs is preferred.
  • Proficiency in Microsoft Office and basic technology applications and Excel.
  • Ability to work independently and collaboratively as part of a multidisciplinary team.

Physical Requirements:

  • Ability to move throughout the building, participate in resident activities, events and provide regular home visits to all residents.
  • Ability to occasionally lift and carry materials weighing up to 20 pounds.
