Summary: Provide case management services in a 19-bed program serving homeless individuals in early recovery. 10 beds are permanent beds targeted for individuals who are chronically homeless and with a diagnosed substance abuse disorder. Provide Needs Assessment and follow up referrals and supportive services to participants in the program. Duties will include intake, case management, daily contact, house meetings, referral, and advocacy. Work to engage client in treatment, refer to programs, and develop/monitor plan to assist in long term recovery.

Primary Responsibilities:

  • Complete assessment with all clients entering the program as assigned.
  • Develop Individual Service Plan with each participant within 2 days of entry into the program.
  • Maintain daily contact with the all clients in the building. Establish and maintain regular contact.
  • Maintain appropriate files and case notes for each participant.
  • Coordinate program duties with co-case manager to ensure all required functions and duties are completed and that effective communication is maintained.
  • Outreach to community providers to market this program and establish contacts for client referrals. Keep detailed log of all referrals made.
  • Co-facilitate weekly house meetings.
  • Ensure accurate statistical data is maintained and reported monthly. Including but not limited to updated records and case notes within Octopia and reports required by funding sources.
  • Attend MHSA meetings as required.
  • Provide crisis intervention, 24-hour on-call coverage.
  • Provide support and direction to resident house managers.
  • Work as an integrated member of the Western Mass Housing and support services team.
  • Provide transportation to clients, as needed.
  • Engage all clients by understanding and addressing their needs whether within or outside the scope of the specialty.
  • Attend & participate in engagement team meetings, follow up with client service plans and communicate effectively with clients and staff in other specialty areas.
  • Ensure compliance with program/department, agency and/or funder requirements, as well as, SMOC policies & procedures.
  • Other duties as assigned. 

Knowledge and Skill Requirements:

  • Bachelor’s Degree or related experience.
  • Previous experience working with homeless or disadvantaged population.
  • Knowledge of 12 step recovery program.
  • Excellent written and oral communication.
  • Must have experience with Word and Excel.
  • Valid driver’s license.

 Organizational Relationship: 

  • Directly reports to Program Director.
  • Direct reports to this position are none.

Physical Requirement: Ability to ascend and descend stairs. Position requires prolonged periods of sitting.

Working Conditions: The program is located in a lodging house. Smoking is not permitted in the building. As part of the responsibilities of this position, the case manager will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services.

Hybrid Work Option: Hybrid work is permissible in some positions at SMOC depending on the key functions and responsibilities. The Case Manager position is eligible to work from home 0-20% of the week in scheduling coordination with the department manager.


Monday through Friday: 8:00AM - 4:00PM. Includes a 1 hour unpaid lunch break.
35 Hours per week.

Skills Required

  • Bachelor's Degree or related experience
  • Previous experience working with homeless or disadvantaged populations
  • Knowledge of 12-step recovery programs
  • Excellent written and oral communication skills
  • Experience with Microsoft Word and Excel
  • Valid driver's license
  • Ability to ascend and descend stairs and tolerate prolonged periods of sitting
