Job Overview 

The IOC Care Manager provides case management services with purposeful interventions to assist in improving treatment adherence and reducing episodes of care in screening services and short-term care facilities. IOC staff will be knowledgeable of the commitment orders for each individual served to provide the team psychiatrist and courts with updated information related to the individual’s adherence to the commitment order and provide information to the psychiatrist regarding needed modification to the commitment order to more effectively address the individual’s mental health needs.

Program Info 

Involuntary Outpatient Commitment (IOC) serves individuals diagnosed with serious, persistent mental illness who, due to his/her mental illness are dangerous to him/herself, others or property in the reasonably foreseeable future. These individuals have also demonstrated a history of positive treatment responses in an outpatient setting. IOC is an opportunity to effect change before an individual reaches a crisis point which results in an inpatient hospitalization.

Responsibilities:

  • Provides face to face case management sessions to clients weekly. Engages family and supports, as appropriate, in treatment planning and stabilization.
  • Oversees client’s discharge transition with collaboration of hospital, compiles treatment plan, conversion interview, and community discharge plan.
  • Completes court documentation packets and commitment paperwork as required.
  • Prepares in advance for summary, tracks court dates, and collaborates with IOC psychiatrist and services provides ahead of court.
  • Collaborate and follow up with current service providers regarding the client history and, present status with treatment involvement.
  • Provide education to the service provider on the role of IOC.
  • On-site presence at hospitals and consistent communication with other community agencies for referrals and pre-discharge planning.
  • Participate in all program, agency, supervision meetings as required.
  • Completes all required documentation: notes within 24hrs of contact, as well as treatment plans and assessments as documented in program manual to support timely billing and required timeframes.
  • Adheres to a flexible schedule based on client and program needs.
  • Meets obligations of the program (contractual, licensing, Medicaid, etc.).
  • Operates conscientiously in accordance with program budget (i.e., use of client engaging funds, office supplies).
  • Performs other related duties as assigned

Requirements:

  • Bachelor's degree in psychology, social work or a related filed, plus 2 years of relevant experience.
  • Must have and maintain a valid driver's license with an acceptable driving record
  • Must be able to exercise independent thinking and good judgment under all circumstances.
  • Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.

Additional Information:

  • Base Rate: $19.23/hourly
  • Sign-on Bonus: $750
  • Schedule: 8:30a-4:30p or 9:00a-5:00p Monday - Friday
  • Bilingual-Spanish differential: $1.50 per hour
