Case Manager II - Targeted Case Management (Dakota County)

People Incorporated is seeking a Case Manager II to join our Targeted Case Management (Dakota County) team to primarily work with clients who are diagnosed with chronic mental illness or other chronic health conditions. We connect people with services through developing person-centered support plans that help them live as independently as possible. Our Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules. 


Hours: Full - Time, Monday - Friday, business hours


Location: Dakota County, Minnesota l Hybrid


Range: $24.25 - $25.75 Hourly, based on qualifications


Hiring Bonus: $1,000 to external candidates

Main Job Duties:

•    Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.
•    Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.
•    Assessing client needs and ensuring continuity of care.
•    Making ongoing referrals to resources based on assessment and current client need.
•    Maintaining necessary daily records for continuity of service and reports to funding agencies.
•    Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards.
•    Attending client monthly meetings to monitor services.

These essential duties include:

Passing all background and requirements for facility and position; and
• The ability to pass the screening required by Federal Human Services Office of Inspector General to assure that person has not been excluded from working in the Health Care Programs which receive federal funding.
• Must have a valid and current driver’s license and auto insurance.

Requirements for All Case Manager II positions:
• Valid driver’s license, excellent driving record and current auto insurance

Education and Experience Required for Targeted Case Manager II position:
• Bachelor’s Degree in behavioral science or related field
• 2,000 hours of supervised experience working with adults with severe and persistent mental illness
OR
• Master’s Degree in behavioral science or related field.

Preferred Qualifications for Targeted Case Manager II position:
• Experience providing direct services to adults with serious and persistent mental illness
• Experience with case management and commitment proceedings
• Knowledge of Recovery, Motivational Interviewing and Person-Centered approach

Skills Required

  • Must be able to have onsite presence within Dakota County, Minnesota
