The Opportunity
Mass Bay Behavioral Health is seeking a dedicated and personable Case Manager to join their Case Management department in Dorchester, MA. This is a full-time, onsite role well-suited for someone who is organized, empathetic, and motivated to help clients navigate complex physical or mental health needs. The ideal candidate enjoys building relationships with clients and their families, connecting people with the right resources, and working alongside a multidisciplinary care team.
What You'll Do
- Work closely with clients and their families to identify needs, goals, and the resources necessary to reach them, helping clients learn to manage their own situations rather than managing it for them
- Conduct intake assessments, including physical screening of clients and belongings, as needed
- Facilitate psycho-educational groups and coordinate individual case management as directed
- Lead treatment team meetings and family sessions
- Reach out to assigned clients to assess urgent needs, appraise situations, and listen to client concerns
- Complete an in-depth biopsychosocial assessment of each client's mental and physical health
- Develop a detailed plan of action for each client, setting goals and identifying the resources needed to meet them
- Provide counseling support in individual or group settings
- Consult with outside agencies to coordinate support services and resources
- Maintain comprehensive records of client progress, including calls, referrals, and home visits
- Transport clients to community referrals using a company vehicle, when needed
- Assist with the collection and processing of urine toxicology screens
- Collaborate with a multidisciplinary team to coordinate clients' medical, financial, and social needs

Requirements
- High School diploma or GED required
- Strong communication, observation, and written documentation skills
- Working knowledge of state and federal patient confidentiality laws
- Understanding of drug-free workplace policies and workplace violence prevention
- Familiarity with corporate integrity, ethics, and the 12-Step Recovery Program
- Knowledge of alcohol and other drugs, including their physical, emotional, intellectual, and spiritual impact on individuals and families
- Ability to recognize a medical emergency and take appropriate action
- Knowledge of emergency procedures for alcohol and/or drug overdose
- First Aid and CPR knowledge; CPR certification required
- Familiarity with federal, state, and municipal regulations governing the treatment of alcoholism and drug addiction, including confidentiality, patient rights, and professional ethics
- Comfort working as part of a multidisciplinary addiction treatment team, with flexibility to adapt to schedule changes
- Familiarity with community resources

- LADC-2 credential
- 3 to 5 years of case management experience in a behavioral health setting

- Compensation: $50,000 to $55,000 annually
