Seeking a Case Manager to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Desert Cove Recovery Health Center.
Full-Time Employment: 40 hours per week
Monday - Friday: 8:00am - 5:00pm

Duties Include:
- Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals.
- Completes assessments, crisis plans and individualized service plans.
- Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health providers, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualized service plan are implemented.
- Conducts outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges to ensure adequate resources are available and in place.
- Performs all case management functions associated with caseload including completing the assessment and service planning processes; including identifying the need for further or specialty evaluations.
- Pursues best practice outcomes for person with mental illness including continuing education, employment, independent housing and community tenure.

- A high school diploma/GED with a minimum of 1.5 years of behavioral health employment experience, or
- An associates degree in the field of medical or behavioral, or
- A bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health experience, or
- A bachelor's degree in the field of behavioral, or
- A master's degree or higher in the field of behavioral health.
- Computer Skills: Proficient in Microsoft Word, Excel, PowerPoint, & Outlook. Must be able to pass typing assessment with minimum 25wpm.
- Knowledge in use of Electronic Health Records - NextGen is a plus.
- Bilingual in Spanish/ASL is desired.
- Excellent oral and written communication skills.
- Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience and meet requirements of Terros Health’s driving policy.
- Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
- Must pass a TB Test.

Skills Required
- High school diploma/GED plus at least 1.5 years of behavioral health employment experience, or an associate degree in medical or behavioral health, or a bachelor's degree outside behavioral health plus one year of behavioral health experience, or a bachelor's degree in behavioral health, or a master's degree or higher in behavioral health.
- Proficiency in Microsoft Word, Excel, PowerPoint, and Outlook.
- Pass a typing assessment at a minimum of 25 words per minute.
- Knowledge of electronic health records; NextGen experience is a plus.
- Excellent oral and written communication skills.
- Valid Arizona driver's license, at least 21 years of age, minimum four years of driving experience, and compliance with the employer's driving policy.
- Valid Arizona Fingerprint Clearance Card or application for a Level 1 card within seven working days of starting.
- Pass a tuberculosis test.
- Bilingual Spanish/ASL skills.
