Position Summary

Crossroads Talent Solutions is seeking a dedicated and detail-oriented Case Manager to support the Louisiana Army National Guard (LAARNG) State Surgeon Office at Camp Beauregard. This position provides comprehensive case management services in support of military medical readiness programs by coordinating medical care, monitoring service member case progression, maintaining accurate records, and collaborating with military healthcare providers and command personnel.

The Case Manager serves as a critical liaison between Soldiers, healthcare providers, and military leadership to ensure timely coordination of medical evaluations, treatment plans, and administrative requirements while maintaining strict confidentiality and compliance with Department of Defense (DoD), Army, HIPAA, and applicable regulatory standards. The solicitation identifies Case Management as a dedicated contract line item under Paragraph 5.3 of the Performance Work Statement.

Essential Duties and Responsibilities
  • Manage assigned medical cases from initial referral through case closure.
  • Coordinate medical appointments, specialty referrals, evaluations, and follow-up care for Soldiers.
  • Track medical readiness requirements and monitor case progress to ensure timely completion.
  • Maintain accurate and complete electronic and paper case records in accordance with Army and DoD policies.
  • Communicate regularly with Soldiers, military treatment facilities, healthcare providers, commanders, and medical staff regarding case status.
  • Review medical documentation for completeness and accuracy.
  • Monitor treatment plans and assist with coordinating recommended services.
  • Identify delays or barriers to care and recommend appropriate solutions.
  • Prepare reports, case summaries, and administrative documentation for leadership.
  • Assist with medical profiling, line-of-duty documentation, and other readiness-related administrative requirements as directed.
  • Maintain strict confidentiality of protected health information (PHI) and personally identifiable information (PII).
  • Ensure compliance with Army regulations, HIPAA, DoD guidance, and organizational policies.
  • Support quality assurance initiatives and assist with continuous process improvement efforts.
  • Participate in meetings, briefings, and case review conferences.
  • Provide outstanding customer service while working with military personnel, healthcare providers, and government stakeholders.
  • Perform additional case management and administrative duties as assigned.
Required Qualifications
  • Bachelor's degree in Nursing, Social Work, Healthcare Administration, Case Management, Public Health, or a related healthcare field preferred.
  • Equivalent combination of education and relevant military or healthcare experience may be considered.
  • Minimum of two to five years of professional case management or healthcare coordination experience.
  • Experience working with military, federal government, or healthcare organizations preferred.
  • Strong knowledge of medical documentation and healthcare terminology.
  • Excellent organizational and time-management skills.
  • Ability to manage multiple cases simultaneously while meeting deadlines.
  • Strong verbal and written communication skills.
  • Proficiency with Microsoft Office Suite (Word, Excel, Outlook, and PowerPoint).
  • Experience using electronic medical record systems and case management databases preferred.
  • Ability to work independently while collaborating effectively with multidisciplinary teams.
  • Demonstrated professionalism, discretion, and sound judgment.
Preferred Qualifications
  • Military healthcare experience.
  • Experience supporting Army Medical Readiness or State Surgeon Office operations.
  • Knowledge of Army medical readiness programs and administrative processes.
  • Certified Case Manager (CCM) or comparable professional certification.
  • Prior experience working with military treatment facilities or federal healthcare programs.

  • May require limited local travel in support of military medical readiness operations.
Physical Requirements
  • Prolonged periods of sitting and computer work.
  • Ability to lift up to 20 pounds on occasion.
  • Ability to move throughout government facilities as required.
