The Case Manager for Adoption provides comprehensive case management and permanency services to children/youth and families as they journey through the adoption process.


  • Provides direct in-home services including, but not limited to, parenting skills, life skills, child development, crisis intervention, and behavior management to children/youth and families.
  • Assesses for safety and completes and reviews safety plans with children/youth and families.
  • Ensures the physical, mental, and educational well-being of children/youth in care are assessed and documented.
  • Completes ongoing assessments by gathering information from children/youth, families, and collaterals.
  • Ensures the social histories for children/youth with a goal of adoption is current.
  • Completes relative kin and adoptive family searches to identify resources for the purposes of adoption.
  • Assists adoptive family with Adoption Placement Agreement and subsidy, finalization and post finalization paperwork and processes.
  • Prepares children/youth and placement families regarding moves, grief, separation, and loss issues related to adoption.
  • Completes an individual recruitment plan according to policy for children/youth who do not have an identified adoption resource.
  • Develops, implements, facilitates, and reviews case plans with children/youth and families, ensuring that permanency goals are reached and activities are achieved within established time frames.
  • Determines and recommends which services are needed for the child/youth and family, including mental health, drug and alcohol services, respite care and supervised or unsupervised visitations/interactions.
  • Assists families to establish a support network of family, kinship, and community service providers.
  • Supports kinship and foster families to maintain placements and help provide stability to children and youth in care.
  • Provides intervention for children/youth and families in crisis situations.
  • Assesses ongoing case plan goals and activities and makes recommendations to the Court.
  • Ability to work with the court system and follow court protocols including appropriate business attire.
  • Completes reports for court according to policy, participates in court proceedings, including testifying, and makes sure court orders are followed.
  • Completes and maintains documentation in the child/youth’s case record accurately and within established time frames.
  • Participates in supervisory and team meetings, sharing pertinent information about children/youth/families’ progress, critical trends, barriers, and solutions to achieving outcomes; and relationships with other agencies/communities.
  • Works collaboratively and communicates weekly with Family Support Worker assigned, providing guidance and support on case decisions.
  • Supervises and transports children/youth and families as needed.
  • On-call rotation where you will be available outside of regular working hours to address any urgent issues that arise during your rotation. The essential functions during on-call periods include:
    • Availability: Ensure that you are reachable via phone or designated communication channels at all times during the on-call period.
    • Rapid Response: Respond to all calls or notifications immediately.
    • Problem Resolution: Diagnose, troubleshoot, and resolve issues promptly. If unable to resolve an issue independently, escalate it to the appropriate Supervisor on-call.
    • Travel: Be prepared to travel short notice to various locations throughout the State as required to address on-site issues. This may include travel to company facilities, client sites, or other relevant locations.
    • Documentation: Record all incidents, actions taken, and resolutions in the designated documentation system or log. Ensure that a clear handover is provided if the issue extends beyond the on-call period.
  • Travels throughout the state of Kansas to provide case management services to children/youth and families, including but not limited to worker/child visits, sibling visits, and pre-adoptive visits.
  • Work a flexible schedule to meet the needs of children, families, and community partners.
  • Implements Saint Francis Ministries policies and follows directives as required. Follows and adheres to all pertinent Saint Francis Ministries Standard Operating Procedures (SOP’s), rules, employee policies and procedures; related accreditation and licensure standards; and federal, state, and local rules, statutes, regulations, and contractual terms.
  • Reports critical incidents to DCF according to policy.
  • Reports unusual incidents through appropriate risk management, clinical and safety channels.
  • Ensures clients’ rights are protected.
  • Is knowledgeable of and follows all safety procedures.
Preferred Qualifications
  • Bachelor’s degree in social work or human service field from an accredited college or university.
  • License to practice in the State of Kansas preferred.
  • Prefer two years’ experience working with children/youth and families.
  • Must be 21 years of age.
  • Must pass a drug screen, MVR, KBI, Child Abuse & Neglect Central Registry Clearance check and an Adult Registry Check.
  • FBI fingerprint check.
  • Lifting Requirements of 50 lbs.
  • Must have a valid driver’s license, acceptable driving record, and reliable transportation.

Skills Required

  • Bachelor's degree in social work or a human service field from an accredited college or university
  • License to practice in the State of Kansas
  • Two years of experience working with children, youth, and families
  • Must be at least 21 years old
  • Pass a drug screen
  • Pass an MVR check
  • Pass a KBI check
  • Pass a Child Abuse and Neglect Central Registry Clearance check
  • Pass an Adult Registry Check
  • Pass an FBI fingerprint check
  • Ability to lift 50 pounds
