Job Summary:

The Providence Center Case Manager ACT Harmony Team will coordinate services for consumers to minimize stressors and to facilitate the reduction of psychiatric symptoms.

Duties and Responsibilities:

Coordinate a variety of services:

Provide supportive counseling, problem solving, and limit setting.

Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services, to include working with people who have sever persistent mental illness and/or substance abuse issues, as it relates to case management.

Monitor client status during spend down process, complete necessary DHS applications.

In cooperation with Managers or Emergency Services Staff, assist with assessments, crisis interventions and risk management as needed.

Negotiate and advocate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.

Transport consumers in own vehicle to planned activities and appointments.

Complete recordkeeping in accordance with Health Information Services (HIS) and other TPC requirements within required time frames and abide by TPC confidentiality policy.

Maintain cooperative relationships with Center staff, consumers, community agencies and the public.

Attend mandatory in-service trainings and other trainings required.

Attend case presentations and conferences.

Other duties as assigned.

Requirements:

Associate's degree required. Bachelor's degree from accredited four-year College or University in Human Services or related field preferred.

Experience in Human Services field preferred.

Bilingual Spanish candidates preferred.

Must have valid driver's license and insured auto.

Must be able to work flexible hours.
