Job Summary:
Under general supervision, the Case Manager provides comprehensive care coordination, service linkage, advocacy, and case management services for individuals receiving behavioral health, substance use disorder, housing, recovery, crisis, and community-based services. This position coordinates behavioral health, medical, social, vocational, housing, and community support services to improve access to care, increase engagement in treatment, and support positive health and recovery outcomes. The Case Manager utilizes a person-centered, trauma-informed, and recovery-oriented approach to service delivery while promoting consumer self-sufficiency, community integration, and wellness.
Essential Functions:
- Coordinates behavioral health, medical, social, vocational, housing, and community support services for assigned individuals.
- Conducts service needs assessments and assists with treatment planning activities.
- Facilitates referrals, service linkages, and transitions of care.
- Assists consumers in accessing benefits, healthcare services, housing resources, employment services, transportation, and community supports.
- Engages consumers and natural support systems in treatment and recovery processes.
- Collaborates with healthcare providers, behavioral health professionals, community agencies, and other stakeholders.
- Monitors service delivery to ensure continuity of care and achievement of treatment goals.
- Coordinates discharge planning and aftercare services.
- Provides advocacy and support to consumers in navigating healthcare and social service systems.
- Assists consumers in identifying and overcoming barriers to treatment, recovery, and community stability.
- Provides crisis support and service coordination when appropriate.
- Maintains compliance with documentation, Medicaid, DBHDD, payer, and agency requirements.
- Maintains confidentiality of consumer information in accordance with HIPAA and organizational policies.
- Completes required documentation accurately and within established timeframes.
- Maintains assigned productivity standards and performance expectations.
- Participates in interdisciplinary treatment team meetings and service planning activities.
- Performs related duties as assigned.

Primary Responsibilities, but not limited to:
Care Coordination
- Coordinates behavioral health, medical, housing, social, vocational, and support services for assigned consumers.
- Assesses service needs and facilitates access to appropriate resources and supports.
- Monitors service delivery and follows up to ensure continuity of care.
Treatment Planning and Service Support
- Participates in treatment planning and service coordination activities.
- Assists consumers in achieving identified treatment and recovery goals.
- Facilitates communication among providers, consumers, and support systems.
Community Resource Development
- Coordinates services with community agencies and provider networks.
- Develops and maintains effective partnerships to improve service access.
- Provides information regarding available services, benefits, and resources.
Crisis Support and Care Transitions
- Assists with crisis response and service coordination activities when needed.
- Facilitates transitions between levels of care and treatment settings.
- Supports discharge planning and aftercare coordination activities.
Documentation and Compliance
- Maintains documentation within required timeframes.
- Monitors service authorizations and compliance requirements.
- Ensures services and documentation meet Medicaid, DBHDD, payer, and agency standards.
Productivity and Quality Management
- Maintains assigned productivity standards and performance expectations.
- Participates in quality improvement activities and program initiatives.
Supports continuous improvement efforts to enhance consumer outcomes and service effectiveness.

Required Education & Experience:
- Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, Sociology, Behavioral Health, Public Health, Healthcare Administration, or a related field.
- Valid Georgia Driver’s License.
- Ability to meet and maintain all position-related qualifications, training requirements, certifications, and competency standards.
Preferred
- Experience in behavioral health, substance use disorder services, case management, care coordination, housing services, healthcare, or human services.
- Experience working with individuals experiencing behavioral health conditions, substance use disorders, homelessness, co-occurring disorders, or other complex needs.
- Experience collaborating with healthcare providers, community agencies, and multidisciplinary treatment teams.
- Knowledge of community resources, benefits systems, and social service networks.
Upon appointment to the position, the employee assumes responsibility for meeting and maintaining all minimum qualification requirements, including applicable certifications, licenses, continuing education, competency validations, and mandatory annual or recurrent training. Work Environment:
- Work is performed in office, clinic, residential, healthcare, home, and community settings.
- Requires frequent interaction with consumers, families, providers, and community partners.
- Requires travel throughout the service area to provide community-based services and support.
- Requires regular use of computers, electronic health records, and standard office equipment.
Physical Demands:
- Frequently walks, sits, stands, drives, and utilizes computers.
- Occasionally climbs, balances, bends, and stoops.
- Frequently lifts and/or moves up to 15 pounds.
- Requires visual acuity sufficient to review documentation and utilize electronic systems.
- Requires effective verbal communication with consumers, families, providers, and community agencies.
- Must be able to travel between Gateway locations and community service sites.
