Pyramid Healthcare is seeking a dedicated Remote Center of Excellence (COE) Case Coordinator to join our growing Client Support team. In this role, you will coordinate care, advocate for clients, facilitate connections to critical health and social services, and support individuals and families throughout treatment and recovery.

As a key member of our multidisciplinary team, you will ensure clients receive coordinated, effective, and compassionate care while helping remove barriers to treatment, recovery, and long-term success.

What You'll Do

Client Care Coordination

  • Coordinate client appointments, schedules, aftercare planning, and treatment-related meetings.
  • Monitor client progress and collaborate with the treatment team to address emerging needs.
  • Develop, facilitate, and monitor individualized care plans.
  • Conduct client and family orientations alongside clinical staff.
  • Provide ongoing case management support to clients and families.

Care Navigation & Advocacy

  • Connect clients with community resources, healthcare providers, transportation services, and aftercare support.
  • Ensure clients have access to a full continuum of traditional and non-traditional services.
  • Assist with referral coordination and provider communication throughout treatment.
  • Support recovery engagement initiatives using assessment data and individualized client needs.

Documentation & Compliance

  • Verify completion of admission documentation, consents, and required records.
  • Maintain accurate client records and documentation in accordance with federal, state, and accreditation standards.
  • Protect confidential client information and maintain HIPAA compliance.
  • Complete required organizational and regulatory trainings.

Cross-Functional Collaboration

  • Communicate regularly with referral sources, community providers, family members, legal representatives, employers, and internal treatment teams.
  • Participate in treatment team meetings and case reviews.
  • Collaborate with outreach and marketing teams to strengthen community provider relationships.
Required Qualifications

Education & Credentials

  • Master's degree from an accredited college or university in Social Work, Psychology, Counseling, Nursing, Chemical Dependency Studies, or a related behavioral health field
  • Addiction counseling certification preferred.
  • Level of Care Assessment experience or ability preferred.

Experience

  • Experience in behavioral health, mental health, substance use disorder treatment, recovery support services, or case management.
  • Experience facilitating individual, family, or process groups preferred.
  • Treatment planning and care coordination experience preferred.
