Supporting the Pennsylvania Center of Excellence (COE)

The Case Manager provides comprehensive case management, care coordination, and recovery support services for individuals and families throughout the treatment continuum. This role conducts assessments, identifies client needs, facilitates engagement in treatment and recovery services, and coordinates access to medical, behavioral health, substance use disorder, and community-based resources. Responsibilities include supporting admissions, treatment planning, discharge and aftercare planning, referral management, family support, and collaboration with internal and external providers to promote continuity of care, positive treatment outcomes, and long-term recovery. The Case Manager maintains regular client contact, monitors progress, provides education and resource navigation, and ensures timelyaccurate documentation in accordance with organizational, regulatory, and program requirements. 

Essential Duties and Responsibilities: 

  • Coordinate client engagement, appointments, treatment schedules, and aftercare planning in collaboration with the treatment team. 
  • Serve as the primary liaison with referral sources, healthcare providers, legal agencies, employers, and other community partners to support client care. 
  • Support the admissions process by ensuring required documentation, consents, medical records, and referrals are completed and maintained. 
  • Conduct client and family orientations alongside the clinical team. 
  • Provide case management services, including transportation coordination, resource referrals, family support, and ongoing client engagement. 
  • Develop, document, and coordinate comprehensive aftercare plans with the treatment team and community providers. 
  • Participate in multidisciplinary treatment team meetings and maintain effective communication with clinical staff. 
  • Complete timely and accurate documentation of all client, family, and provider interactions in EHR. 
  • Must exercise discretion and maintain confidentiality with regard to all company information 
  • Completion of all required trainings as designated by the company and accreditation/licensing entities. 
  • Other duties as assigned. 

Additional Essential Duties and Responsibilities for Center of Excellence (COE) programs: 

In addition to the responsibilities listed above, Case Managers assigned to Center of Excellence (COE) programs are responsible for: 

  • Proactively identifying, engaging, and supporting individuals with Opioid Use Disorder (OUD), including those newly diagnosed or not yet connected to recovery services. 
  • Conducting comprehensive assessments, including Social Determinants of Health (SDoH), to identify clinical, behavioral health, recovery, and community support needs. 
  • Providing education on Medication-Assisted Treatment (MAT) options, treatment availability, and recovery support services. 
  • Developing, implementing, and monitoring individualized care plans in collaboration with the Recovery Support Services Team and interdisciplinary treatment providers. 
  • Coordinating access to medical, behavioral health, substance use disorder treatment, recovery services, and community resources across the full continuum of care. 
  • Assisting clients with navigation of insurance benefits, healthcare systems, transportation, community supports, and other barriers to treatment and recovery. 
  • Facilitating warm handoffs and maintaining collaborative relationships with external providers, referral sources, and community partners to ensure continuity of care. 
  • Monitoring client progress, tracking engagement milestones, and adjusting care coordination strategies to support successful treatment outcomes. 
  • Conducting level-of-care assessments, when appropriate and within scope of practice. 
  • Utilizing assessment findings to support Recovery Specialist engagement strategies and improve client outcomes. 
  • Leading or participating in interdisciplinary team meetings to coordinate services, address identified needs, and escalate concerns as appropriate. 
  • Collaborating with Marketing and Business Development teams to strengthen referral relationships and community partnerships. 
  • Maintaining accurate, timely, and compliant documentation, case notes, and encounter data in accordance with COE, organizational, and regulatory requirements. 
  • Supporting ongoing care coordination and recovery-focused services to improve client engagement, retention, and long-term recovery outcomes. 
  • Performing additional COE-related duties and responsibilities as assigned. 
Required Qualifications

Education & Credentials

  • Master's degree from an accredited college or university with a major in Chemical Dependency, Psychology, Social Work, Counseling, Nursing (with a clinical specialty in a human services field), or another related field.  
  • Full certification as an addictions counselor by a statewide certification body that is a member of a national certification body, or certification by another state's substance abuse counseling certification board preferred.   

Experience

  • Experience facilitating process groups, individual therapy, and family therapy preferred.  
  • Drug and Alcohol and/or Mental Health experience preferred.  
  • Treatment planning experience preferred.
