Ampla Health is seeking a CalAIM Clinical Consultant (RN) to provide clinical oversight and consultation for CalAIM, Care Coordination, Referral Center, Call Center, and Transportation services. This position evaluates members’ care management needs, supports individualized care plans, and provides clinical guidance to staff to ensure appropriate, coordinated patient care.
Key Responsibilities- Provide clinical oversight for comprehensive assessments and individualized care plans.
- Evaluate patient needs and recommend appropriate care management services.
- Provide clinical consultation and guidance to CalAIM Program Managers, Lead Case Managers, Care Coordinators, Referral Center, Call Center, and Transportation staff.
- Assist with identifying and managing high-risk patients.
- Provide guidance regarding post-hospital care, referrals, care coordination, and appropriate levels of service.
- Support patient-centered care and help reduce unnecessary healthcare utilization.
- Maintain effective communication, professionalism, and confidentiality.
- Perform other duties as assigned.
- Current California RN License required; BSN preferred.
- Current CPR certification required.
- Minimum typing speed of 40 WPM.
- Ability to use Microsoft Word and standard computer applications.
- Strong clinical, organizational, communication, and problem-solving skills.
- Ability to communicate effectively with providers, nursing staff, patients, and the public.
- Ability to adapt to various environments and responsibilities.
- Knowledge of Ampla Health’s clinic structure, standards, procedures, and protocols preferred.
- Bilingual English/Spanish, English/Punjabi, or English/Hmong preferred but not required.
- Ability to lift up to 40 pounds and push up to 100 pounds on wheels.
- Ability to sit, stand, bend, stoop, crouch, reach, kneel, and twist/turn.
- Adequate vision, hearing, and manual dexterity for daily job functions.
- May be exposed to contagious/infectious diseases.

- Ability to hear, speak clearly, and communicate with clients and staff (telephone and in person)
- Ability to lift up to 40 pounds and push up to 100 pounds (on wheels)
- Adequate vision to read memos, computer screens, registration forms, and other clinic documents
- High manual dexterity and ability to bend, squat, sit, stand, stoop, crouch, reach, kneel, twist/turn
