POSITION SUMMARY:

Reporting to the on-shift Charge Nurse or DON, the certified nursing assistant is responsible for performing daily resident care activities, in his/her scope of practice, of an assigned group of residents, documenting care given. 

This position description is intended to identify some of the duties and responsibilities of this position. MODOC MEDICAL CENTER reserves the right to modify, supplement, delete or augment the duties and responsibilities specified in this position description, at MODOC MEDICAL CENTER’S sole and absolute discretion.

POSITION RESPONSIBILITIES:

General Competencies:

  1. Always demonstrates a professional behavior, both when on duty and as a representative of the hospital outside of normal work hours.

  3. Demonstrates organizational ability and time management.

  5. Demonstrates the ability to compile and organize data using Microsoft Office Applications. Ability to make appropriate recommendations or conclusions, given the data obtained.

  7. Communicates appropriately, respectfully, and clearly to directors, managers, and coworkers. Accepts direction as provided without questioning authority, within the chain of command.
  8. Performs all assigned tasks accurately.
  9. Takes direction from others to include those employees of other departments with which he/she works on a temporary basis in accordance with the chain of command.
  10. Demonstrates ability to effectively use office machines in the performance of job functions.
  11. Answers phone calls, assists public, and forwards appropriately if necessary in a professional and friendly manner.
  12. Performs other duties as assigned.

Specific Competencies:

  1. Delivers restorative care to residents according to established protocol.
  2. Works in conjunction with other team members to build a team.
  3. Demonstrates personal attributes which enhance the management of the unit thus the care of each resident.
  4. Fulfills responsibility to the facility by adhering to its philosophy and general policies.
  5. Performs or assists with personal hygiene on residents following established procedures including: bed bath, tub, shower, sitz bath, oral care, back care, pericare, skin care, nail and foot care, and A.M./H.S. care.
  6. Monitors and records the resident’s vital signs accurately and according to established procedures including: temperature, pulse, respirations, blood pressure, and pain scale.
  7. Performs or assists with treatments or procedures ordered by the physician including: ace bandage, ted hose, non-sterile dressing changes, application of K-pad, cold packs and other treatments as assigned.
  8. Assists residents with activity and transfer including: position change, dangling, ambulation, R.O.M., use of walker, cane, crutches, hoyer lift, prosthetic devises and gait belt use.
  9. Assists residents with food and nutrition including: passes trays, provides set-up assistance, assists residents to eat, supplies nourishments, passes water, charts food percentages and intake/output accurately.
  10. Obtains specimens from residents using established procedures.
  11. Admits residents to the facility according to established procedures and assists in making them and their families oriented and comfortable.  Discharges residents from the facility according to established procedure.
  12. Cares for residents in isolation following established procedures. Abides by Standard precautions.
  13. Assists in meeting residents and their family’s emotional, psychological and social needs while in the facility. Notifies the charge nurse when problems arise.
  14. Helps support residents and their families during and after death and does post mortem care according to established procedure.
  15. Observes safety rules while transporting residents per wheelchair, gurney, etc. Uses gait belt and Hoyer lift appropriately when ambulating and /or transferring residents who require it. Reports any equipment problems or safety hazards to the charge nurse.
  16. Charts care given and observations made accurately for long-term care residents following established procedures.
  17. Straightens and tidies up resident units, tub/shower areas, hallways and utility areas as needed. Assures that resident personal items are well marked and not co-mingled in resident rooms. Assures that potentially harmful items are locked up as per policy.
  18. Assists team members as necessary in completing work assignments, endeavors to work with team members to build a spirit of team work. Knows and uses established channels of communication.
  19. Participates in ward discussions and/or resident care planning regarding resident care issues. Offers suggestions for improved resident care.
  20. Performs additional duties as requested or assigned by the charge nurse or nursing administration.

Professional Requirements:

  1. Adheres to dress code; appearance is neat and professional.
  2. Completes annual education requirements if applicable.
  3. Maintains regulatory requirements.
  4. Wears identification while on duty.
  5. Attends annual evaluation and participates actively in this process.
  6. Reports to work on time and as scheduled; completes work in designated time.
  7. Attends all meetings as appropriate.
  8. Exhibits the mission, ethics and goals of Modoc Medical Center in the performance of job duties
  9. Performs other duties, as assigned. 

WORKING CONDITIONS:

  1. Willingness to work beyond normal working hours and in other positions temporarily when necessary.
  2. Is involved with personnel, visitors, and government agencies, etc., when necessary.
  3. Must function independently, have personal integrity, have flexibility and the ability to work effectively with other personnel, clients and support agencies.
  4. Hours are determined based on needs of the department. Shifts may be eight, ten or twelve hour shifts as agreed upon by the DON and CNO.  

PHYSICAL REQUIREMENTS:

  1. Sits, stands, bends, lifts, walks and moves intermittently during working hours.
  2. Able to lift up to 35 pounds without assistance.
  3. Able to be on feet majority of the day. 
  4. Able to push/pull equipment, carts, wheelchairs, beds, etc.
  5. Able to hear call lights and respond accordingly. 
  6. You must know and use proper body mechanics to avoid injury to yourself and others.
Qualifications

POSITION QUALIFICATIONS: 

  1. High School diploma preferred or equivalency exam.
  2. Current C.N.A. licensure preferred; if non-certified, must become certified within four months of hire.
  3. Current BLS level CPR certification.
  4. Able to communicate in English both verbally and in writing.  
  5. Is dependable, reliable with good attendance and the ability to work with others, at all levels within the organization, and collaborate effectively.
  6. Above-average interpersonal, problem-solving, and written and oral communication skills.
  7. Must be a team player to all departments, confidential of sensitive data, respectful of all other staff members, considerate of other people’s time, and accountable for work output.
  8. Previous clinical experience training and experience in a nursing home/ hospital setting desirable.

Skills Required

  • Current C.N.A. licensure or obtain certification within four months of hire

  • Ability to communicate in English verbally and in writing
