The Role
Provides recovery-oriented case management and consumer support for individuals in community settings. Responsibilities include coordinating healthcare, benefits, housing, and service providers; conducting support contacts; developing safety and service plans; maintaining clinical records and documentation; locating unreachable clients; participating in treatment meetings; completing required paperwork and training; and fulfilling rotating on-call duties. Regular travel to clients’ homes and community locations is required.

As a Blended Case Manager, you will play a meaningful role in supporting clients throughout Lehigh and Northampton Counties, helping them move toward greater stability, independence, and well‑being. This position calls for strong communication and problem‑solving skills, along with the ability to engage clients directly in their home and community environments. 


Responsibilities include:

  • Rehabilitation & Consumer Support
    Demonstrate a recovery-oriented attitude, use a consumer-driven and strength-based approach, assist with basic living needs, and ensure annual physicals and medical monitoring.
  • Case Management & Service Coordination
    Act as primary case manager, conduct minimal bi-weekly support contacts, complete service/safety plans, coordinate benefits and housing, and follow through with clinical decisions.
  • Consumer Tracking & Documentation
    Locate consumers when unreachable, secure past records, develop and update social histories and clinical summaries, maintain accurate service-use summaries, and complete re-authorization paperwork.
  • Collaboration & Team Participation
    Coordinate services with providers, participate in treatment team meetings and service plan meetings with families and providers, act as liaison with supports, and attend all required meetings and trainings.
  • Professionalism & Administrative Duties
    Maintain professionalism per policy, complete paperwork accurately and on time, and fulfill on-call duties in the assigned rotation.

This position is community-based and requires travel to homes and other community locations to provide services and support clients. Mileage reimbursement provided.

Earn $20.07 per hour in the full-time (40 hr ) role. 

*An additional $2 per hour is available with the selection of our Enhanced Pay option! *

Preferred Qualifications

A qualified candidate will possess the following qualifications:

  • Case Management or similar experience preferred
  • High school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science, and 2 years' experience in public or private human services with 1 year in direct care required or a bachelor's degree in sociology, social welfare, psychology, gerontology, or other social science
  • Valid Driver's license and clean driving record required, Vehicle for travel required. 
  • Must be able to work Monday-Friday 8 am - 4:30pm

Skills Required

  • High school diploma plus 12 semester credit hours in sociology, social welfare, psychology, gerontology, or another social science, and two years of public or private human services experience including one year in direct care
  • Alternatively, a bachelor's degree in sociology, social welfare, psychology, gerontology, or another social science
  • Valid driver's license and clean driving record
  • Vehicle available for travel
  • Availability to work Monday through Friday, 8:00 a.m. to 4:30 p.m.
  • Case management or similar experience
