Responsibilities:

  • Promote a rehabilitation and recovery-focused, consumer-driven, strength-based approach to case management and crisis services.
  • Serve as primary case manager for assigned consumers, ensuring bi-weekly support contacts in appropriate settings.
  • Develop and maintain consumer-driven service and safety plans, coordinate benefits, housing, and all aspects of service delivery.
  • Locate consumers through family or community contacts when necessary.
  • Collaborate with service providers, monitor progress, and maintain accurate records of hospitalizations, crises, and incarcerations.
  • Gather and update social history and clinical summaries annually for each consumer.
  • Participate in treatment team, service plan, and provider meetings; follow through on clinical decisions using problem-solving skills.
  • Assist consumers with basic living needs, including housing, food, healthcare, transportation, socialization, education, and employment.
  • Ensure annual physicals and monitor medical health through documentation.
  • Complete reauthorization paperwork promptly and attend all meetings, supervision, and training professionally.
  • Act as liaison with families, providers, and community supports; maintain professionalism per corporate policy.
  • Complete all documentation accurately and on time; fulfill on-call duties as assigned.

This position is Community-based and requires travel to homes, and other community locations to provide services and support clients. Mileage reimbursement provided.

Earn $22.85 per hour*

        *Additional $2 per hour available with the selection of our Enhanced Pay option!

Preferred Qualifications
The ideal candidate will possess one the following qualifications:
  • A high school diploma plus a minimum of 12 college credit hours in Psychology, Sociology, Social Welfare, Gerontology required and 2 years of behavioral health direct care experience required; 

                -or-

  • A Bachelor’s degree with major course work in sociology, social work, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; 

The candidate will have all of the following:

  • Must possess a valid driver's license, insurance and have a clean driving record.
  • Own a reliable vehicle.
  • Must be able to work 9am-5pm Monday-Friday-OPTIONAL PAID ON-CALL AVAILABLE!

Skills preferred 

  • Experience in case management or related human services field preferred.
  • Strong organizational and communication skills.
  • Ability to work independently and manage a flexible schedule.
  • Commitment to person-centered care and community-based service delivery.

Skills Required
