The WIC Coordinator interfaces daily with TrueCare health center patients and staff to enhance recruitment, retention, and re-engagement in the Women, Infants and Children (WIC) Program. This position promotes the value and visibility of WIC services by identifying patients who may be WIC eligible, facilitating routine referrals, and conducting targeted face-to-face in-reach to encourage WIC program enrollment and ongoing participation.
Additionally, the WIC Coordinator is responsible for determining WIC program eligibility of applicants and participants and will complete thorough nutrition and health assessments during certification and recertification appointments, providing basic nutrition education and breastfeeding support, prescribe and issue food benefits, participate in outreach and breastfeeding activities, and provide referrals to other TrueCare services and community programs.

Duties & Responsibilities:
•Reviews the daily schedule for Women’s Health Services (WHS) and the Pediatric department in the electronic health record system (EPIC) to identify WIC eligible individuals to initiate face-to-face in-reach to encourage WIC program enrollment and ongoing participation.
•Attends and participates in WHS and Pediatric team huddles and department meetings to collaborate and share WIC updates with health center staff.
•Collaborates with health center staff to coordinate and secure WIC eligible medical formulas and breast pumps for WIC participants in accordance with policies and procedures.
•Accurately determines WIC program eligibility in accordance with policies and procedures using the WIC Web Information System Exchange (WIC WISE).
•Assesses individual nutritional status and identifies appropriate nutrition risks and interventions by reviewing biochemical (hemoglobin/hematocrit), anthropometric (length/height and weight), dietary, and health data in accordance with policies and procedures.
•Completes thorough nutrition and health assessments during certification and recertification appointments providing basic nutrition education and breastfeeding support using Participant Centered Education (PCE) principles and techniques; refers high-risk (HR) participants to the Degreed Nutritionist (DN) or Registered Dietitian (RD) for HR counseling and follow up in accordance with policies and procedures.
•Provides ongoing nutrition and breastfeeding education to WIC participants (via phone, video, or in person) based on their Nutrition Education Plan (NEP); accurately documents Care Plans, Nutrition Education Contacts (NECs), and pertinent comments/alerts in WIC WISE.
•Accurately prescribes and issue WIC food benefits; maintains card stock security.
•Independently manages and maintains assigned WIC WISE appointment templates and schedule; accurately schedules WIC appointments in accordance with department scheduling guidelines and in alignment with participants’ Nutrition Education Plans (NEPs).
•Utilizes TrueCare’s Electronic Medical Records (EMR) system and CDPH/WIC approved systems to accurately assess and manage WIC participant data in compliance with confidentiality and privacy policies and procedures.
•Promotes and supports breastfeeding as the recommended method of infant feeding in alignment with CDPH/WIC, TrueCare, and Breastfeeding Friendly Health Center guidelines.
•Assists with the Electric Breast Pump Loan Program, including pump distribution, documentation, and cleaning.
•Refers participants to other TrueCare services and community programs; documents follow-up as needed.
•Supports WIC Program Administration through caseload maintenance activities, participation in workgroups, and outreach efforts.
•Attends and actively participates in WIC Department Meetings, WIC huddles, continuing education and trainings, as assigned.
•Flexibility to work at assigned WIC location(s) based on department needs and/or goals.
•Ensure punctual, regular, and timely attendance in accordance with the TrueCare Employee Handbook; report to work on time and as scheduled and remain at work for the entire duration of each shift; and follow TrueCare policies for notifying the supervisor of and documenting all planned and unplanned absences.
 

Required Qualifications:
•High school diploma or equivalent.
•Minimum of six (6) months of experience as a Certified WIC Nutrition Assistant.
•Strong typing and computer skills with proficiency in Microsoft Office applications (Outlook, Word, Excel, PowerPoint, and Teams).
Desired Qualifications:
•Associate’s degree from an accredited institution in nutrition or another health-related field.
•Certified Lactation Educator (CLE) or Certified Lactation Counselor (CLC) Credential
•Bilingual English/Spanish.
The pay for this role starts at $25.30 on an hourly basis.

Skills Required

  • High school diploma or equivalent
  • Knowledge of WIC program requirements
  • At least two years' customer service experience in a healthcare setting
  • Fluent in speaking, reading, and writing English and Spanish
  • Computer proficiency with data entry and Microsoft Office (Outlook, Word, Excel, PowerPoint)
  • Experience providing breastfeeding support and promoting breastfeeding
  • Knowledge of available health and human services/resources within the community
