We are seeking a dedicated and knowledgeable Asthma Education Specialist to join our team on a per diem basis. This role involves delivering home-based asthma education to patients under the age of 18 with uncontrolled asthma. Working collaboratively with a Community Health Worker and the Asthma Program Coordinator, the Asthma Education Specialist will play a critical role in improving patient outcomes in the Greater Waterbury, Greater Torrington, and Naugatuck Valley regions.
Key Responsibilities:
Provide comprehensive asthma education and self-management training to pediatric patients and their families, in accordance with National Heart, Lung, and Blood Institute (NHLBI) asthma guidelines.
Conduct home visits to assess environmental triggers and provide tailored recommendations to reduce exposure.
Collaborate with a Community Health Worker and Asthma Program Coordinator to develop and implement individualized asthma action plans.
Educate patients and caregivers on proper medication use, including inhaler techniques and adherence to prescribed therapies.
Document patient progress, interventions, and outcomes in accordance with organizational and regulatory standards.
Participate in ongoing training and professional development to remain current with best practices in asthma care and management.
Qualifications:
Education: Registered Nurse (RN), Certified Asthma Educator (AE-C), or Respiratory Therapist (RRT) with a focus on asthma care.
Experience: Demonstrated experience in asthma education, management, and patient care, preferably in home or community-based settings.
Knowledge: In-depth understanding of NHLBI asthma guidelines and best practices in asthma self-management.
Skills: Strong interpersonal and communication skills, cultural competence, and the ability to work effectively with diverse populations.
License/Certification: Current licensure or certification in relevant fields (e.g., RN, RRT, AE-C).
Transportation: Reliable transportation to conduct home visits across the service regions.
Work Schedule: This is a per diem position with flexible hours, depending on patient needs and scheduling requirements.
