Job Summary
The AVP, Clinical Product & Strategy will set the strategic direction for clinical programs and products that create measurable value for Medicare Advantage and Traditional Medicare populations served by CenterWell Home Health (CWHH). This work will directly impact the experience of more than 400,000 patients across the 37 states in which we operate.
This leader will translate enterprise priorities into scalable home-based care models that improve quality, reduce avoidable medical costs, strengthen risk adjustment accuracy, and deepen member/patient engagement.
Reporting to the VP, West Region President & Clinical Strategy, this role will lead a team of ~20 associates to partner across Home, Pharmacy, Primary Care, and Humana to identify opportunities, build business cases, launch pilots, and scale programs that support CWHH’s nationally scaled home health business and broader integrated care strategy.

Key Responsibilities


·         Own MA value creation strategy: Develop the roadmap for clinical programs that improve HH and MA Stars performance, affordability, risk adjustment, and member outcomes.


·         Oversee clinical program execution: Provide day-to-day oversight of strategic clinical programs, including performance management, issue resolution, and cross-functional coordination.


·         Improve Quality and Stars performance: Lead gap closure and quality improvement efforts for CWHH populations, partnering with clinical, analytics, population health, and MA teams.


·         Drive appropriate Home Health utilization: Expand appropriate use of Home Health services and demonstrate the associated network savings, member impact, and operational requirements.


·         Reduce avoidable medical costs: Scale interventions that reduce hospitalizations, readmissions, emergency department utilization, and other avoidable costs.


·         Advance member engagement and longitudinal care: Build programs that improve outreach, participation, adherence, and ongoing care management beyond a single Home Health episode.


·         Expand high-risk member services: Identify and develop services for high-risk members outside the traditional Home Health episode, including new home-based care models and pilots.


·         Improve ecosystem integration: Strengthen clinical integration across CWHH, PCO, Rx, and Humana through shared workflows, data, care pathways, and operating routines.

Qualifications

  • Bachelor’s degree in healthcare, business, or related field required; Master’s degree preferred, not required.
  • A minimum of 10 years of progressive healthcare leadership experience
  • A minimum of 7 years leading managers and professional teams

Work at Home Requirements: To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.
Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


 

$203,400 - $279,800 per year
