The Clover Home Care team is responsible for ensuring patients have access to the right care at the right time, in the right setting. We are a group of caring and passionate professionals dedicated to improving the lives of our most vulnerable members by providing in-home primary care, annual wellness visits, supportive care services, behavioral health and community services coordination.
As an Associate Practice Manager, you will support the Senior Practice Manager and play a crucial role in overseeing administrative, care coordination, and field support staff to ensure quality and timely patient care. You will help establish, implement, and manage standard workflows and procedures to improve efficiency and effectiveness within the practice.
This is a hybrid position, with in-person presence required as directed by the Senior Practice Manager and through self-directed collaboration at our Lyndhurst office.
As an Associate Practice Manager you will:
- Assist in overseeing administrative, care coordination, and field support teams to ensure timely and high-quality patient care.
- Support the implementation and management of standardized workflows and procedures to enhance operational efficiency and effectiveness.
- Collaborate closely with the Medical Director and Senior Practice Manager to achieve practice goals and key performance metrics.
- Work cross-functionally with other departments and teams within Clover to ensure smooth practice operations.
- Assist in managing staffing, scheduling, training, and performance management for all areas under your responsibility.
- Participate in individual and team meetings to review progress, identify challenges, and develop strategies to achieve key business goals and metrics.
- Monitor reports and dashboards to identify trends and uncover opportunities for improvement.
- Ensure compliance with policies, procedures, regulatory requirements, billing protocols, and medical records administration.
- Develop and demonstrate expertise with the EMR and other systems to effectively support practice management.
- Facilitate the resolution of patient issues and concerns.
- Support the operations of additional home care programs and take on other projects and responsibilities as assigned.
Success in this role looks like:
In the first 90 days, you'll complete onboarding, understand your responsibilities, assess workflows, and start building strong team relationships while ensuring compliance and addressing immediate issues. By 6 months, your focus will shift to optimizing workflows, managing staff effectively, and achieving key performance indicators (KPIs) for patient care and operational efficiency. Long-term success involves sustaining improvements, leading strategic initiatives, fostering team development, and consistently achieving high patient satisfaction while adapting to changes in healthcare.

A reasonable estimate of the base salary range for this role is $84,800 - $130,000. Final pay is based on several factors including but not limited to internal equity, market data, and the applicant’s education, work experience, certifications, etc.
