Assisted Living Coordinator – Spring Oak | Tri-Cities Area

Spring Oak is seeking an Assisted Living Coordinator to join our team in the Tri-Cities area and help lead our community in delivering exceptional care and service.

Responsibilities

  • Recruit, interview, hire, welcome, and onboard Wellness Staff in coordination with the Director of Health and Wellness and Human Resources. 
  • Prepare and maintain Wellness Staff schedules, manage call-outs and open shifts, and assist with ensuring appropriate daily staffing coverage. 
  • Support employee retention and satisfaction through regular communication, new-hire follow-up, employee recognition, in-services, training coordination, and assistance with routine employee concerns. 
  • Serve as a point of contact for residents and families, address routine questions and concerns, coordinate family meetings, and ensure appropriate follow-up.  
  • Support resident satisfaction and retention by remaining visible within the community, identifying concerns, and coordinating with appropriate departments to resolve service issues.  
  • Conduct resident assessments and reassessments when permitted by state regulations and within the employee's qualifications and scope of practice.  
  • Assist with developing, reviewing, and updating resident service plans and help ensure approved services are communicated and delivered appropriately.  
  • Assist with new resident move-ins, hospital and rehabilitation returns, transfers, discharges, and other resident transitions 
  • Report changes in resident condition, service concerns, staffing issues, and other significant matters to the Director of Health and Wellness.  
  • Assist the Director of Health and Wellness with departmental documentation, reports, audits, meetings, training, records, supplies, and other administrative or operational needs.  
  • Fill in for Wellness Staff as needed and perform direct resident care or medication-related duties within the employee's license, certification, training, and scope of practice.  
  • Assist during emergencies, evacuations, falls, staffing shortages, and other community situations as needed.  
  • Maintain confidentiality and comply with HIPAA, resident rights, company policies, and applicable regulatory requirements.  
  • Perform other related duties and responsibilities as assigned by the Director of Health and Wellness or Executive Director.  

Qualifications

  • Must be a Certified Medication Aide or Registered Medication Aide, Licensed Practical Nurse, social worker, or other experienced healthcare or senior living professional.
  • Any required license, certification, or registration must be current and in good standing in the state of employment.
  • Must have prior experience in assisted living, senior living, healthcare, resident services, staffing, or a related field.
  • Experience with recruiting, interviewing, scheduling, onboarding, or training employees is preferred.
  • Must possess strong communication, organization, problem-solving, and follow-through skills.
  • Must have the ability to interact professionally and tactfully with residents, families, employees, medical providers, and the general public.
  • Must have the ability to use computerized scheduling, payroll, EMAR, electronic medical record, and other applicable systems.
  • Must be willing and qualified to provide direct resident care or medication assistance as needed and within the employee's license, certification, training, and scope of practice.

Skills Required

  • Certified Medication Aide or Registered Medication Aide, Licensed Practical Nurse, social worker, or other experienced healthcare or senior living professional
