The patient case manager assistant is a non-clinical position with customer service experience responsible for assisting the case manager in maintaining the quality of patient care in accordance with the agencies objectives and policies are adhered to.

DUTIES AND RESPONSIBILITIES:

The duties and responsibilities include, but may not be limited to:

  • Provides supervision of nurses and home health aides to ensure the quality of services and maintenance of standards.
  • Reviews and investigates the clinical content, diagnosis, medical history, medications and other procedures of the record of patients under their supervision.
  • Directs staff toward the appropriate use of community resources.
  • Appraises performance at probation and annually with the Director of Nursing. Provides the teaching and counseling necessary to enhance the delivery of services and professional development of nursing professional and health aide staff.
  • Identifies and evaluates nursing needs of the patient and appropriateness of home care during supervisory visits.
  • Assists with the orientation of new professional staff to agency policies and procedures.
  • Sees patients in the home setting as needed.
  • Participates in audits, interdisciplinary team conferences and represents agency in the community as requested.
  • Reviews nurse’s schedule for recertifications, discharges and home health aide supervisory visits.
  • Reviews weekly schedule done by nurse for changes or updates.
  • Ensures HIPPA guidelines and procedures are maintained.

Regulatory Requirements:

  • High School Diploma or Equivalent

Language Skills:

  • Able to communicate effectively in English, both verbally and in writing.
  • Additional languages preferred

Professional Requirements:

  • Knowledge Medicare and Medicaid guidelines
  • Excellent communication skills and ability to establish good rapport with other staff members.  
  • Working knowledge of home health care and the principles and techniques of professional nursing and required documentation thereof.
  • Skillful in organization of work and in principles of time management; and knowledge of management process.  
  • Ability to contribute to the quality of care being rendered through constructive communication with nursing managers and nursing staff.
  • Bilingual English and Spanish or Creole preferred.

Physical Requirements:

The physical requirements of this position include lifting, prolonged sitting.  The mental/sensory requirements include recall, reasoning, problem-solving, hearing, speaking clearly, writing legibly, keyboarding, reading and logical thinking. The environment's pace can vary from steady to a sometimes-fast pace requiring handling of multiple priorities, frequent, sometimes intense customer interactions.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


 

$38,000 - $45,800 per year
