We are seeking an After-Hours Nurse who will provide skilled nursing and hospice care to maximize the comfort and health of patients.
MUST BE BILINGUAL IN CANTONESE
Schedule:
Tuesday & Friday; 3:30pm - 12:00am
Saturday & Sunday; 8:30am - 5:00pm
Location:
San Francisco County with coverage as needed to Marin County
By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.
Essential Duties & Responsibilities include (but are not limited to):
- Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
- Under the guidance of the After-Hours Nurse, responsible for patients/families including assessing, planning, implementing, and evaluating nursing services consistent with the plan of care.
- Communicate with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/ family.
- Maintain regular communication with Team Leaders regarding patient/family status and unusual or potentially problematic patient/family issues.
Education/Experience:
- Associate's or Bachelor’s degree in nursing from an accredited School of Nursing
- Experience in acute care/ home health and/or hospice setting preferred.
Certificates, Licenses, Registrations:
- Current license as a registered professional nurse in the State of California.
- Public Health Nursing certification highly desirable.
- HPNA certification preferred
Pay Range:
$64.50 - $78.70 shift differential is included

Skills Required
- Current license as a Registered Professional Nurse in the State of California
- One year experience as a professional nurse in the past three years or Bachelor's degree in Nursing from an accredited School of Nursing
- Experience in acute care, home health, and/or hospice setting
- Public Health Nursing certification
- HPNA certification
- Compliance with CDPH vaccine requirements for healthcare personnel
- Ability to work specified after-hours schedule and travel to cover San Francisco and Marin counties as needed
