We are seeking an After-Hours Nurse who will provi­de skilled nursing and hospice care to maximize the comfort and health of patients.

MUST BE BILINGUAL IN CANTONESE

Schedule:

Tuesday & Friday; 3:30pm - 12:00am

Saturday & Sunday; 8:30am - 5:00pm

Location:

San Francisco County with coverage as needed to Marin County

By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.
Essential Duties & Responsibilities include (but are not limited to):

  • Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
  • Under the guidance of the After-Hours Nurse, responsible for patients/families including assessing, planning, implementing, and evaluating nursing services consistent with the plan of care.
  • Communicate with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/ family.
  • Maintain regular communication with Team Leaders regarding patient/family status and unusual or potentially problematic patient/family issues.

Education/Experience:

  • Associate's or Bachelor’s degree in nursing from an accredited School of Nursing
  • Experience in acute care/ home health and/or hospice setting preferred.

Certificates, Licenses, Registrations:

  • Current license as a registered professional nurse in the State of California.
  • Public Health Nursing certification highly desirable.
  • HPNA certification preferred

Pay Range:
$64.50 - $78.70 shift differential is included

Skills Required

  • Current license as a Registered Professional Nurse in the State of California
  • One year experience as a professional nurse in the past three years or Bachelor's degree in Nursing from an accredited School of Nursing
  • Experience in acute care, home health, and/or hospice setting
  • Public Health Nursing certification
  • HPNA certification
  • Compliance with CDPH vaccine requirements for healthcare personnel
  • Ability to work specified after-hours schedule and travel to cover San Francisco and Marin counties as needed
