- As an Admission Nurse, you will serve as the first clinical point of contact for patients and families, ensuring a smooth transition into home health services while delivering exceptional clinical assessments and support.
- Conduct comprehensive admission assessments to identify patients' physical, psychosocial, and environmental needs
- Complete OASIS assessments accurately and timely
- Develop and implement individualized plans of care that support positive patient outcomes
- Perform initial nursing evaluations and ongoing reassessments as appropriate
- Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care
- Communicate significant patient condition changes and clinical findings promptly
- Ensure accurate, timely, and compliant documentation in the medical record
- Maintain patient privacy and confidentiality in accordance with HIPAA regulations
- Identify and report safety concerns and implement appropriate interventions
- Support quality care initiatives and organizational standards
- Perform additional duties and projects as assigned
- Graduate of an accredited nursing program
- Current, unrestricted Registered Nurse (RN) license in the state of practice
- Minimum one (1) year of Home Health experience
- Strong OASIS knowledge and documentation experience
- Valid driver's license, reliable transportation, and current auto insurance
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Strong clinical assessment, communication, and organizational skills
