RATE OF PAY:
RN - $36 up to $44 an hour, PLUS On top of those great base rates we offer additional shift differentials for evening and weekend shifts!!

RESPONSIBILITIES:
- Oversees the MDS process following the RAI manual
- Works with multiple other MDS nurses to complete the initial assessments for new admissions.
- Supervised by the DON to ensure the MDS nurses properly manage the scheduling of patient admissions (comprehensive assessment, order entry, initiating care plans and ensuring the patient’s has the proper equipment at the time of admission), MDS assessment and care planning completion
- Completes the required submissions weekly or as designated by the Director of Nursing. During submissions will review the validation report to ensure no errors with submission
- Maintains ability to obtain and review QM’s. Works with IDT to initiate improvement projects to improve resident outcomes
- Attends the IDT Medicare meeting (understands skilled criteria) and Medicaid meeting to collaborate with the IDT to complete the MDS process.
- Develops/updates comprehensive care plan and nurse aide care plan (Kardex) in conjunction with the MDS to accurately address the needs of the resident.
- Oversees that non-nursing sections of MDS and CAA’s are accurate representations of resident needs.
- Coordinates, manages and monitors the care plan and identifies problems/needs of the resident and goals by reviewing the order recap report and the nurses’ notes to assure the care plan is being followed and to see if another OBRA MDS assessment may need to be completed
- Monitors resident for change of status to determine need for significant change per RAI
- Ensures supportive documentation is accurate according to MDS/RAI regulations.
- Works with facility consultants as necessary and implement recommended changes as required.
- Completes required paperwork/AIMS, Fall Risk scale, Braden, Progress note, Care Plan, nurse aide care plan (Kardex)/.
QUALIFICATIONS
- Must have current RN license for the state in which the facility is located.
- Needs to be flexible with her / his working schedule and able to work independently
- Requires an understanding of the MDS process and a willingness to continue to learn and remain current with MDS updates set by CMS.
- Prefer knowledge in PDPM (skilled reimbursement) and RUG (Long Term Care reimbursement)

Skills Required
