Job Overview 

This position is responsible for actively maintaining current network market share and expanding new network opportunities for the Transplant ProgramAs the primary point of contact for the program, this position provides information, education and support to providers and the programs network of referring providersSupports greater integration of outpatient and transplant follow upImplements systems and processes to recruit new patients and referring providers, improve patient selection, review outcomes, provide early and accurate feedback to referring cliniciansTracks performance data and conducts regular site visits to review results with physician and staff, helps to create, implement and follow-up on action plans to further improve patient outcomes and efficiency of evaluation process. 

 

Job Description 

Minimum Qualifications: 

1. Bachelor of Science in Nursing (BSN). 

2. Massachusetts RN Licensure. 

3. Basic Life Support (BLS) Certification. 

4. Two (2) years of kidney transplantation experience. 

 

Preferred Qualifications: 

1. Masters degree in Nursing Education. 

2. Advanced Cardiac Life Support (ACLS) Certification. 

3. Clinical Transplant Coordinator (CCTC) Certification. 

4. Five (5) years of kidney transplantation experience. 

5. Extensive RN experience. 

 

Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive listOther duties and responsibilities may be assigned. 

 

1. Serves as the Tufts Medical Center Transplant Nursing liaison to TMIN and other physicians for transplant related matters. 

2. Maintains detailed current nursing and clinical knowledge of transplant process, patient population and regulatory environment. 

3. Utilizes tools developed to monitor performance. 

4. Facilitates collaborative relationships among the various disciplines and departments to ensure effective quality patient care delivery. 

5. Provides consultation to all new patients and family members regarding evaluation transplant evaluation process and follow up requirements. 

6. Participates in department administrative meetings. 

7. Conducts weekly transplant multi-disciplinary meetings. 

8. Facilitates all transplant evaluations and presents them at multidisciplinary meetings. 

9. Maintains current referral sources and building new relationship. 

10. Provides frequent feedback to referring clinicians about the status of their patients within our program. 

11. Tracks referral volumes and network relationship. 

12. Actively seeks new information and maximizes creative options to help aid in ongoing learning needs of all members of referral network. 

 

Physical Requirements:  

1. Prolonged, extensive, or considerable standing/walking. 

2. Lifts, positions, pushes and/or transfer patients and equipment. 

3. Considerable reaching, stooping, bending, kneeling, crouching. 

4. Frequent exposure to hazardous chemicals, sick patients, bodily substances, noise and possible exposure to radiation, lasers, electric shock, etc. 

5. Regularly exposed to the risk of bloodborne diseases and other transmissible infections. 

6. Has contact with patients under a wide variety of circumstances. 

7. Subjected to varying and unpredictable situations. 

8. May have contact with hazardous materials. 

9. Subjected to irregular hours. 

 

Skills & Abilities: 

1. Ability to effectively communicate with patients, families, physicians, and healthcare team. 

2. Strong computer skills, including proficiency with MS Office (Word, Excel and Power Point), and familiarity with databases including Transplant Medical Record Systems. 

3. Knowledgeable in CMS and UNOS regulations. 

4. Possesses and applies the skills and knowledge necessary to provide care to patients throughout the life span, with consideration of aging processes, human development stages and cultural patterns in each step of the care process. 

5. Ability to provide care for a designated group of patients according to unit guidelines and hospital policies, procedures and protocols. 

6. Ability to interpret age specific data and response to care and provide age-appropriate communication.  

7. Ability to handle emergency and crisis situations.

Pay Range:


$101,084.26 - $128,878.21
